即刻种植修复上前牙的美学效果及对骨量和牙周组织健康的影响

摘　　要：目的 探究即刻种植修复上前牙的美学效果及对骨量和牙周组织健康的影响。方法 选取在我院口腔科行上前牙种植修复的96例患者,根据种植时机将其分为延期组、早期组和即刻组,各32例。延期组行延期种植术,早期组行早期种植术,即刻组开展即刻种植术。比较三组的种植成功率、美学效果、唇侧骨量、骨量吸收率、牙周组织健康程度。结果三组的种植成功率无显著差异(P>0.05)。延期组的近中龈乳头、牙龈高度、牙槽骨外形、远中龈乳头评分及PES总分低于即刻组,早期组的近中龈乳头、牙槽骨外形、远中龈乳头评分以及PES总分低于即刻组,延期组的PES总分低于早期组(P<0.05)。即刻组种植术后即刻及种植术后6个月的唇侧骨量低于延期组和早期组,骨量吸收率高于延期组和早期组(P<0.05)。三组修复后1、6个月的PD无显著差异(P>0.05)。结论 不同种植时机对上前牙缺损的种植成功率和牙周组织健康无显著影响,但即刻种植的美学效果更佳,延期种植和早期种植在维持患者唇侧骨量稳定性方面的效果较好。Objective To explore the aesthetic effect of immediate implant in the restoration of upper anterior teeth and its influences on bone mass and periodontal tissue health.Methods Ninety-six patients who underwent upper anterior dental implant restoration in stomatology department of our hospital were selected.According to the timing of implanting,the patients were divided into delayed group,early group and immediate group,with 32 cases in each group.The delayed group performed delayed implanting,the early group performed early implanting,and the immediate group performed immediate implant.The implantation success rate,aesthetic effect,labial bone mass,bone mass absorption rate and periodontal tissue health degree were compared among the three groups.Results There was no significant difference in the implantating success rate among the three groups(P>0.05).The scores of mesial gingival papilla,gingival height,alveolar bone shape,distal gingival papilla and PES total score in the delayed group were lower than those in the immediate group,the scores of mesial gingival papilla,alveolar bone shape,distal gingival papilla and PES total score in the early group were lower than those in the immediate group,and the PES total score in the delayed group was lower than that in the early group(P<0.05).The labial bone mass of the immediate group was lower than that of the delayed group and the early group immediately after implanting and 6 months after implanting,and the bone mass absorption rate was higher than that of the delayed group and the early group(P <0.05).There was no significant difference in PD among the three groups at 1 and 6 months after restoration(P>0.05).Conclusion Different implanting timing has no significanting effect on the implanting success rate and periodontal tissue health of upper anterior tooth defect.But the immediate implant has better aesthetic effect,and delayed implanting and early implanting are better in maintaining the stability of patients’ labial bone mass.
