摘 要:目的:探讨调强放疗(intensity modulated radiotherapy,IMRT)与三维适形放疗(three dimensional conformal radiotherapy,3DCRT)在子宫颈癌根治术后放疗中的应用。方法:回顾性选取2019年1月-2021年7月中国医科大学航空总医院收治的84例子宫颈癌根治术后辅助放疗患者的临床资料。根据放疗技术的不同将其分为3DCRT组和IMRT组,分别为41、43例。3DCRT组采用3DCRT,IMRT组采用IMRT。比较两组放疗后靶区适形指数(conformity index,CI)和均匀指数(homogeneity index,HI),膀胱、直肠、小肠、双侧股骨头V_(15)、V_(30)、V_(45),放疗期间不良反应。结果:放疗后,IMRT组CI及HI均优于3DCRT组,差异有统计学意义(P<0.05)。放疗后,3DCRT组直肠V_(45),膀胱V_(30)及V_(45),小肠V_(30)及V_(45),双侧股骨头V_(15)、V_(30)、V_(45)均高于IMRT组,差异均有统计学意义(P<0.05)。IMRT组膀胱、肠道、血液系统不良反应发生率均低于3DCRT组,差异均有统计学意义(P<0.05)。结论:与3DCRT相比,子宫颈癌术后IMRT技术有更好的靶区剂量分布,可降低危及器官受照射体积及不良反应发生风险。Objective:To investigate the application of intensity modulated radiotherapy(IMRT)and three-dimensional conformal radiotherapy(3DCRT)in radiotherapy after radical resection of cervical cancer.Method:The clinical data of 84 patients with adjuvant radiotherapy after radical resection of cervical cancer treated in Aviation General Hospital of China Medical University from January 2019 to July 2021 were retrospectively selected.According to different radiotherapy techniques,the patients were divided into 3DCRT group and IMRT group,with 41 and 43 patients respectively.The 3DCRT group used 3DCRT,and the IMRT group used IMRT.The conformity index(CI)and heterogeneity index(HI)in the target area,V_(15),V_(30),V_(45)of bladder,rectum,small intestine and bilateral femoral head after radiotherapy,and adverse reactions during radiotherapy were compared between the two groups.Result:After radiotherapy,CI and HI in IMRT group were better than those in 3DCRT group,the differences were statistically significant(P<0.05).After radiotherapy,V_(45)of rectum,V_(30)and V_(45)of bladder,V_(30)and V_(45)of small intestine,V_(45),V_(30)and V_(45)of bilateral femoral head in 3DCRT group were higher than those in IMRT group,the differences were statistically significant(P<0.05).The incidences of adverse reactions in bladder,intestine and blood system in IMRT group were lower than those in 3DCRT group,the differences were statistically significant(P<0.05).Conclusion:Compared with 3DCRT,IMRT has better target dose distribution after cervical cancer surgery,and can reduce the volume of organ at risk irradiation and the risk of adverse reactions.
