癌性骨痛膏外敷控制骨转移癌痛的临床研究

摘　　要：目的:探讨癌性骨痛膏外敷联合阿片类药物治疗骨转移癌痛的临床疗效。方法:2019年11月-2020年11月甘肃中医药大学附属医院肿瘤内科收治的80例伴有中度或中度以上骨转移癌痛患者,采用随机数字表法分为观察组和对照组各40例。两组均按滴定剂量给予奥施康定,观察组同时用癌性骨痛膏贴敷,疗程均为7 d,观察治疗前后两组患者的止痛疗效、生活质量、阿片类药物不良反应发生情况等。结果:治疗后观察组患者的总有效率(90.0%,36/40)显著高于对照组(65.0%,26/40),差异有统计学意义(P<0.05)。治疗后观察组患者的生活质量改善率(62.5%)显著高于对照组(40.0%),差异有统计学意义(P<0.05)。两组患者便秘发生率差异有统计学意义(P<0.05)。结论:骨转移癌痛患者在阿片类药物治疗基础上,使用癌性骨痛膏贴敷能够减轻骨转移癌痛患者疼痛,缓解患者便秘症状,提高患者的生活质量,具有重要临床研究意义,值得进一步推广应用。Objective:To explore the clinical efficacy of external application of Cancer Gutong ointment(骨痛膏)on the control of bone metastasis cancer pain.Methods:A total of 80 patients with metastatic bone pain(numerical rating scale[NRS]≥4))selected from November 2019 to November 2020 in the Department of Oncology,Affiliated Hospital of Gansu University of Traditional Chinese Medicine were randomly allocated to the observation group(40 cases)and the control group(40 cases).Both groups were given OxyContin according to the dosage titration,and the observation group received external application of Cancer Gutong ointment at the same time.The NRS score,Quality of Life(QOL)scales and opioids adverse reactions of two groups were evaluated after 7 days.Results:After treatment,the total effective rate of pain treatment in the observation group(90.0%,36/40)was significantly higher than that of the control group(65.0%,26/40)(P<0.05).After treatment,the improvement rate of quality of life(62.5%)in the observation group was significantly higher than that of the control group(40%)(P<0.05).There was statistically significance(P<0.05)between different groups in terms of constipation.Conclusion:The external application of Cancer Gutong ointment can relieve metastatic bone pain,reduce adverse reactions of OxyContin,relieve symptoms of patients with constipation,and improve their QOL,it has important clinical significance and is worthy of further popularization and application..
