外用丁酸氢化可的松乳膏缓解鲜红斑痣激光术后反应的临床研究

摘　　要：目的:评价激光术后即刻外用丁酸氢化可的松乳膏对减轻脉冲染料激光治疗鲜红斑痣术后反应的疗效。方法:选取鲜红斑痣患者384例,随机分为试验组(192例)及对照组(192例)。试验组激光术后即刻外用丁酸氢化可的松乳膏;对照组激光术后即刻外用丁酸氢化可的松的基质霜。所有患者均给予单次脉冲染料激光治疗,于术后第3、7、30天进行随访,观察患者术后疼痛、肿胀、红斑、结痂及水疱等情况。结果:术后疼痛评分试验组和对照组分别为3.47±0.68,4.90±1.63,差异有统计学意义(P<0.05);术后红斑评分两组分别为2.65±0.58,2.82±0.64,差异有统计学意义(P<0.05);红斑持续时间两组分别为(7.59±2.44)d,(9.17±3.01)d,差异有统计学意义(P<0.05);术后肿胀评分两组分别为1.77±0.73,2.53±1.01,差异有统计学意义(P<0.05);肿胀持续时间两组分别为(1.80±1.64)d,(2.83±1.79)d,差异有统计学意义(P<0.05),对照组1例术后出现明显瘢痕。结论:外用丁酸氢化可的松乳膏可有效减轻脉冲染料激光治疗鲜红斑痣的术后疼痛及术后反应,可临床推广应用。Objective To evaluate the efficacy of hydrocortisone butyrate cream in the post-operative reaction, which induced by pulsed dye laser irradiation for port wine stain. Methods A total of 384 patients with port wine stain were enrolled in this study and randomly assigned into two regimens:treatment group(192) and control group(192).All patients were given a single treatment of pulsed dye laser. The patients in treatment group received hydrocortisone butyrate cream immediately, and were followed-up at day 3, 7 and 30. Results The VAS scores were 3.47±0.68,4.90±1.63(P<0.05), in the treatment group and control group respectively. The erythema scores were 2.65±0.58, 2.82±0.64(P<0.05), in the treatment group and control group respectively.The duration of erythema lasted for(7.59±2.44) days,(9.17±3.01) days(P<0.05), respectively. The swell scores were 1.77±0.73,2.53±1.01(P<0.05),respectively.The duration of erythema lasted for(1.80±1.64) days,(2.83±1.79)days(P<0.05).In the control group, 1 patient left scar after treatment. Conclusion Topical hydrocortisone butyrate cream can significantly reduce post-operative reaction induced by pulsed dye laser for the treatment of port wine stain.
