摘 要:目的:探讨臭氧油抗菌剂治疗慢性牙周炎的疗效及对龈下菌群的影响。方法:按照随机数字表法将2019年1月-12月笔者医院口腔科收治的200例慢性牙周炎患者分为对照组和观察组,每组100例。对照组患者给予龈下刮治术,观察组患者在此基础上加用臭氧油抗菌剂,治疗4周。比较治疗前及治疗后6周、12周两组的菌斑指数(PLI)、牙龈指数(GI)、探诊深度(PD)和附着丧失(AL);于治疗前及治疗后6周、12周对龈下菌斑16S rDNA进行PCR后行琼脂糖凝胶电泳,分析菌群种类及含量。结果:治疗后6、12个月,观察组的PLI、GI、PD、AL均低于对照组,差异均具有统计学意义(P<0.05);治疗前,观察组与对照组分别发现27种与28种菌群,治疗后6、12个月,观察组与对照组分别发现5种与9种菌群,观察组的牙龈卟啉单胞菌、伴放线菌嗜血菌、福赛斯坦纳菌、中间普雷沃菌、齿垢密螺旋体含量低于对照组,差异均具有统计学意义(P<0.05)。结论:臭氧油抗菌剂治疗慢性牙周炎的疗效明显,对慢性牙周炎致病菌具有很好的杀灭作用。Objective To discuss the clinical efficacy of ozone oil antibacterial agent on chronic periodontitis and its effects on subgingival microflora. Methods According to the random number method, 200 patients with chronic periodontitis who were divided into control group and observation group in the stomatology department of our hospital from January to December 2019,100 patients in each group. Patients were treated with subgingival curetry in the control group, and patients were treated with ozone oil antibacterial agent on the subgingival curetry in the observation group, all the patients were treated for 4 weeks.The plaque index(PLI), gingival index(GI), probing depth(PD), and attachment loss(AL) was compared between two groups before and 6, 12 weeks after treatment. After PCR on subgingival plaque 16S rDNA, agarose gel electrophoresis was performed to analyze the species and contents of bacteriabefore and 6 and 12 weeks after treatment. Results At 6 and 12 months after treatment, PLI, GI, PD and AL in the observation group was all lower than those in the control group, with statistically significant differences(P<0.05). Before treatment, 27 and 28 strains of bacteria were found in the observation group and the control group,respectively. 5 and 9 strains were found in the observation group and the control group at 6 and 12 months after treatment, respectively. The contents of Porphyromonas gingiva, Haemophilus actinomycete,Forsystana,Prevotella intermediate and Treponema pallidus in the observation group were lower than those in the control group, with statistically significant differences(P<0.05). Conclusion Ozone oil antibacterial agent was effective in the treatment of chronic periodontitis and it had a good killing effect on the pathogenic bacteria of chronic periodontitis.
