摘  要:目的 探讨蛛网膜下腔出血(SAH)患者血清白细胞介素-6(IL-6)、C-反应蛋白(CRP)、S100及脑脊液NO变化对脑血管痉挛及预后的影响。方法 对45例SAH患者均进行了Hunt-Hess分级评定。根据DSA检查将患者分为脑血管痉挛(CVS)组和非CVS组;根据APACHE II评分将患者分为预后良好组和预后不良组。分别检测血清中IL-6、CRP、S100和脑脊液中NO水平,并进行统计学分析。结果 Hunt-Hess分级等级越高,血清IL-6、CRP、S100及脑脊液NO表达水平越高;CVS组和预后不良组血清中IL-6、CRP、S100和脑脊液中NO均有较高表达,差异有统计学意义(P<0.05)。脑血管痉挛和预后不良与血清中IL-6、CRP、S100和脑脊液中NO表达水平均呈正相关,差异有统计学意义(P<0.05)。结论 SAH患者血清中IL-6、CRP、S100和脑脊液中NO表达水平升高,与SAH后的CVS发生及预后不良呈正相关,可作为SAH早期临床干预治疗的参考。objective To investigate the effects of serum interleukin-6(IL-6),C-reactive protein(CRP),S100 and cerebrospinal fluid NO changes on cerebral vasospasm and prognosis in patients with subarachnoid hemorrhage(SAH).Methods Forty-five patients with SAH were evaluated by Hunt-Hess grading.According to DSA examination,they were divided into the CVS group and the non-CVS group.According to APACHE II score,SAH patients were divided into the group with good prognosis and the group with poor prognosis.The levels of IL-6,CRP,S100 and NO in cerebrospinal fluid were detected respectively,followed by statistical analysis of correlation.Results The higher the Hunt grade was,the higher the expression of serum interleukin-6(IL-6),c-reactive protein(CRP),S100 and NO in cerebrospinal fluid;the expression of IL-6,CRP,S100 in serum and NO in cerebrospinal fluid were higher in the CVS group and the poor prognosis group,and the difference was statistically significant(P<0.05).Cerebral vasospasm and poor prognosis were positively correlated with the expression levels of IL-6,CRP,S100 and NO in cerebrospinal fluid,and the differences were statistically significant(P<0.05).Conclusion The expression levels of IL-6,CRP,S100 and NO in cerebrospinal fluid in SAH patients have significantly increased,showing positive correlations with cerebral vasospasm and poor prognosis after SAH,which provides a new direction for early clinical intervention of SAH.
