急性脑梗死患者血清miR-15b、miR-19a和VEGF水平与颈动脉狭窄程度的相关性分析

摘　　要：目的:探讨急性脑梗死(ACI)合并颈动脉颅外段狭窄患者血清血管内皮生长因子(VEGF)、胎盘生长因子(PLGF)、半胱氨酸蛋白酶-3(Caspase-3)水平的检测意义。方法:选取2020年1月-12月在本院确诊的130例ACI患者,根据CT血管造影(CTA)有无颈动脉颅外段狭窄分为ACI合并颈动脉颅外段狭窄组(n=85)和ACI无颈动脉颅外段狭窄组(n=45),根据颈动脉狭窄程度分为轻度狭窄组(n=62)、中度狭窄组(n=35)和重度狭窄组(n=33),选取同期健康体检者110例作为正常对照组。采用酶联免疫法检测血清VEGF水平,采用实时荧光定量PCR检测血清miR-15b、miR-19a表达。分别比较ACI组、对照组及轻度、中度及重度颈动脉颅外段狭窄组血清VEGF水平和miR-15b、miR-19a的相对表达量;Pearson相关分析血清VEGF水平和miR-15b、miR-19a相对表达量与ACI患者颈动脉颅外段狭窄程度的相关性。结果:ACI合并颈动脉颅外段狭窄组血清VEGF水平和miR-15b、miR-19a相对表达量明显高于ACI无颈动脉颅外段狭窄组和正常对照组(P<0.05);重度狭窄组患者血清VEGF水平和miR-15b、miR-19a相对表达量明显高于中度狭窄组和轻度狭窄组(P<0.05);血清VEGF水平和miR-15b、miR-19a相对表达量与ACI患者颈动脉颅外段狭窄呈显著的正相关(P<0.05)。结论:ACI患者血清VEGF水平和miR-15b、miR-19a相对表达量与颈动脉颅外段狭窄程度有关。Objective:To explore the detection significance of serum vascular endothelial growth factor(VEGF),placental growth factor(PLGF)and cysteine?protease-3(Caspase-3)levels in patients with acute cerebral infarction(ACI)and carotid extracranial stenosis.Methods:A total of 130 patients confirmed with ACI in the hospital were enrolled between January and December 2020.According to presence or absence of carotid extracranial stenosis by CT angiography(CTA),they were divided into ACI with extracranial carotid stenosis group(n=85)and ACI without extracranial carotid stenosis group(n=45).According to the severity of carotid stenosis,they were divided into mild stenosis group(n=62),moderate stenosis group(n=35)and severe stenosis group(n=33).And other 110 healthy controls during the same period were enrolled as normal control group.The level of serum VEGF was detected by enzyme-linked immunosorbent assay.The expressions of serum miR-15b and miR-19a were detected by real-time fluorescent quantitative PCR.The level of serum VEGF and relative expression levels of miR-15b and miR-19a in ACI group,control group,mild,moderate and severe stenosis groups were compared.The correlation between serum VEGF,miR-15b,miR-19a and the severity of carotid extracranial stenosis was analyzed by Pearson correlation analysis.Results:The serum VEGF level and the relative expression of miR-15b and miR-19a in ACI with extracranial carotid stenosis group were significantly higher than those in ACI without extracranial carotid stenosis group and normal control group(P<0.05).The serum VEGF level and relative expression of miR-15b and miR-19a in severe stenosis group were significantly higher than those in moderate stenosis group and mild stenosis group(P<0.05).Serum VEGF level and relative expression levels of miR-15b and miR-19a were significantly positively correlated with carotid extracranial stenosis in ACI patients(P<0.05).Conclusion:The serum VEGF level and the relative expression of miR-15b and miR-19a in ACI patients are related to the degree o
