摘　　要：目的 探讨动脉溶栓联合介入治疗老年急性脑梗死患者的疗效及其对脑血流动力和脑血管储备(cerebrovascular reserve,CVR)能力的影响。方法 选取2017年6月-2020年6月于乐山市人民医院脑血管病科治疗的104例老年急性脑梗死患者,按照治疗方法不同随机分为两组,对照组和研究组各52例,其中对照组实施常规治疗,研究组实施动脉溶栓与介入联合治疗,评估其治疗前后NIHSS评分、治疗疗效、脑血流动力和CVR能力。结果治疗后两组患者的NIHSS、DVp和DVm均较治疗前下降,Vp、Vm明显增高,研究组改变更为显著(P<0.05)。研究组疗效分布及临床有效率均优于对照组;与治疗前相比较,治疗后两组患者PI明显下降、CVR明显增高,研究组改变更明显(P<0.05)。结论 动脉溶栓联合介入治疗可稳定缺血脑组织,有利于改善脑组织缺血状态。Objective To investigate the effect of arterial thrombolysis combined with interventional therapy in treatment of elderly patients with acute cerebral infarction and its effect on cerebral blood flow and cerebral vascular reserve. Methods 104 elderly patients with acute cerebral infarction from June 2017 to June 2020 in our hospital were randomly devided into control and study group, 52 cases in each. The control group was treated with conventional treatment, and the study group was treated with intra-arterial thrombolysis and interventional therapy. NIHSS score, therapeutic effect, cerebral hemodynamics, and cerebrovascular reserve capacity were evaluated before and after treatment.Results After treatment, NIHSS, DVp and DVm of the two groups were decreased, Vp and Vm were significantly increased, and the changes in the study group were greater(P<0.05). The distribution of curative effect and clinical effective rate of the study group were better than those of the control group. Compared with pre-treatment, Pi of the two groups was decreased significantly, while CVR was increased significantly, and the study group changed more(P<0.05).Conclusion Arterial thrombolysis combined with interventional therapy can stabilize ischemic brain tissue and improve ischemic state of brain tissue.
