摘 要:目的监测与分析宫颈恶性肿瘤患者的凝血指标变化,研究其凝血功能特点,为临床诊疗提供实验室依据。方法选取195例宫颈恶性肿瘤患者为宫颈恶性肿瘤组,另选取同期240例宫颈良性肿瘤患者为宫颈良性肿瘤组,收集同期203例健康体检者为健康对照组。采用Stago全自动血凝仪检测全部受试对象的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、凝血酶时间(TT)、D二聚体(D-D)水平,并进行比较。结果三组PT、TT、FIB、D-D水平比较,差异有统计学意义(P<0.05)。宫颈恶性肿瘤组PT、TT、FIB、D-D水平与健康对照组比较,差异有统计学意义(P<0.05),与宫颈良性肿瘤组TT、FIB、D-D水平比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示:D-D(OR=2.949,95%C71.828,4.757,P<0.05)为宫颈恶性肿瘤的独立危险因素。结论宫颈恶性肿瘤会引发体内凝血状态的改变,及时进行凝血指标的检测,对宫颈恶性肿瘤的诊断具有重要预测价值。Objective To monitor and analysis the characteristics of blood coagulation function in patients with cervical cancer by analyzing the changes of coaqulation indexes in patients,so as to provide laboratory basis for clinical diagnosis and treatment.Methods 195 cases of cervical malignant tumor patients were selected as cervical malignant tumor group,another 240 cases of cervical benign tumor patients were selected as cervical benign tumor group,203 cases of healthy physical examination during the same period were collected as healthy control group.The prothrombin time(PT),activated partial thrombin time(APTT),fibrinogen(FIB),thrombin time(TT)and D-dimer(D-D)levels of all subjects were detected by Stago automatic hemagglutination apparatus and compared.Results PT,TT,FIB and D-D levels were significantly different among the three groups(P<0.05).The levels of PT,TT,FIB and D-D in cervical malignant tumor group were significantly different from those in healthy control group(P<0.05),and the levels of TT,FIB and D-D in cervical benign tumor group were significantly different from those in healthy control group(P<0.05).Multivariate Logistic regression analysis showed that D-D(OR=2.949,95%CI1.828,4.757,P<0.05)was an independent risk factor for cervical cancer.Conclusion Cervical cancer can trigger a change in the clotting state in our body,to detect coagulation indexes in time have important predictive value for the diagnosis of cervical cancer.
