探讨阴道镜活检诊断宫颈高级别鳞状上皮内病变的价值及影响诊断准确率的相关因素

摘　　要：目的探讨采取阴道镜活检诊断宫颈高级别鳞状上皮内病变(HSIL)的价值及影响诊断准确率因素。方法回顾性分析160例HSIL患者的临床资料,患者均采取阴道镜活检及术后病理检查,以术后病理结果作为金标准,评价阴道镜活检对诊断HSIL的价值及影响诊断准确率的相关因素。结果阴道镜活检诊断准确142例,准确率为8&75%,漏诊18例。多因素Logistic回归分析显示:年龄>50岁、阴道分娩史、宫颈病变面积<50%及无宫颈管搔刮均是影响阴道镜下HSIL诊断准确率的主要因素。结论阴道镜活检对诊断HSIL的准确率高,但也存在一定漏诊情况,临床在实际中需规避影响诊断准确率的相关因素,以提高HSIL诊断准确率。Objective To investigate the value of colposcopy biopsy in the diagnosis of high-grade squamous intraepithelial lesion(HSIL)and the factors affecting the diagnostic accuracy.Methods The clinical data of 160 patients with HSIL were retrospectively analyzed.All patients underwent colposcopic biopsy and postoperative pathological examination.The postoperative pathological results were used as the gold standard to evaluate the value of colposcopic biopsy in the diagnosis of HSIL and the related influencing factors affecting the diagnostic accuracy.Results Colposcopy biopsy was accurate in 142 cases(88.75%)and missed in 18 cases.Multivariate Logistic regression analysis showed that age>50 years old,vaginal delivery history,cervical lesion area<50% and no cervical canal scratching were the main influencing factors for the diagnosis accuracy of HSIL under colposcopy.Conclusion Colposcopy biopsy has a high accuracy in the diagnosis of HSIL,but there are also certain missed diagnosis,which causes the need to avoid the relevant factors affecting the diagnostic accuracy in practice and improve the diagnostic accuracy ofHSIL.
