摘  要:目的利用静息态功能磁共振技术探讨月经周期变化对无先兆偏头痛的影响,了解其神经病理机制,为未来探索新的治疗靶点。方法从四川大学华西医院偏头痛数据库纳入符合条件的27例月经相关性无先兆偏头痛和26例非月经期无先兆偏头痛患者,对所有受试者进行临床症状采集、量表评分和头颅功能磁共振成像检查,分析组间差异性脑区并将其标准化低频振幅值与临床症状和量表评分进行线性相关分析。结果月经相关性无先兆偏头痛组患者在多个脑区出现活动显著增强或减弱,并与头痛持续时间或HIT-6量表评分呈线性相关(P<0.05,高斯随机场校正)。结论月经相关性无先兆偏头痛存在多个脑区功能紊乱,这些异常的脑区可能参与其神经病理学发病机制,并有望在将来成为治疗的参考靶点。Objective To explore the effect and neuropathological mechanism of menstrual cycle on migraine without aura by resting state functional magnetic resonance imaging,so as to explore new therapeutic targets in the future.Methods 27 menstrual-related migraine without aura(MRM)patients and 26 non-menstrual migraine without aura patients from the migraine database of West China Hospital were included.All subjects were evaluated for clinical symptoms and scale scores and functional magnetic resonance imaging.Linear correlation analysis was conducted between difference brain regions and clinical symptoms and scale scores.Results The MRM group showed enhanced or weakened activities in multiple brain regions,which were linear correlated with duration of headache attack or HIT-6 results(P<0.05,Gaussian random field correction).Conclusion There are multiple brain regions with functional disorders in MRM,which may participate in its neuropathological pathogenesis and are expected to prove targets for the treatment of MRM patients in the future.
