摘 要:目的采用度中心度(DC)和格兰杰因果分析(GCA)方法研究早期基底节区缺血性卒中(EISB)患者脑功能网络有效连接变化,分析其与认知功能障碍的关系。方法对27例EISB患者组及27例健康对照组(HC)行静息态功能磁共振成像扫描,选取组间DC值有统计学差异的脑区作为种子点进行全脑体素GCA分析;比较两组一般认知功能差异;提取GCA显著差异脑区功能连接值,分析其与MoCA评分的相关性。结果EISB组MoCA评分低于HC组,差异有统计学意义(t=-7.33,P<0.05);与HC组比较,EISB组左侧颞下回及海马DC值增加(P<0.05,FDR校正),EISB组左侧颞下回到全脑连接效应减弱的脑区为左侧中央前后回,EISB组全脑到左侧颞下回连接效应增强脑区为左侧中央前后回,EISB组全脑到左侧颞下回连接效应减弱的脑区为左侧前扣带回;GCA显著差异脑区功能连接值与MoCA评分之间没有相关性(P>0.05)。结论EISB患者在认知学习网络、记忆网络、感觉运动网络、额顶网络、默认模式网络等多个脑网络间出现有效功能连接异常改变,提示认知功能障碍患者在早期即存在全脑多功能网络损伤。Objective To explore the abnormalities of effective connectivity of functional network and the relationships with cognitive dysfunction in patients with early ischemic stroke in the basal ganglia(ESBI)groups.Methods Resting state functional magnetic resonance imaging(rs-fMRI)was performed in 27 patients with ESBI and 27 healthy controls.Brain regions with statistically different DC values between groups were selected as seed points for GCA analysis.The differences of MoCA score between the two groups were compared.The functional connectivity values of GCA were extracted,and the correlation between them and MoCA score was analyzed.Results The MoCA score of ESBI group was lower than that of HC group(t=-7.33,P<0.05);Compared with HC group,the DC value of the left inferior temporal gyrus and hippocampus in ESBI group increased(P<0.05,FDR correction).In ESBI group,a reduced effective connectivity with the left inferior temporal gyrus to the left anterior-posterior central gyrus was founded,a increased effective connectivity with the left anterior-posterior central gyrus to the left inferior temporal gyrus was founded,a reduced effective connectivity with the left anterior cingulate gyrus to the left inferior temporal gyrus was founded.No correlation was found between GCA effective connection value and MoCA score(P>0.05).Conclusion The abnormal changes of effective functional connectivity among cognitive learning network,memory network,sensorimotor network,frontoparietal network,default mode network and other brain networks in patients with ESBI suggest that patients have multi-functional network damage in the whole brain in the early stage.
