摘 要:目的通过凝集素芯片技术,分析I、II、III、IV期肺癌患者与正常健康志愿者血清糖蛋白糖链谱的差异,同时发现与肺癌发生发展相关的糖链结构标志物,为更早地发现及鉴别肺癌提供临床依据。方法首先利用凝集素芯片技术,比较分析南华大学附属第二医院2017年9月~2018年10月收治的不同分期肺癌54例患者(早期即Ⅰ期和Ⅱ期9例,Ⅲ期24例,Ⅳ期21例)以及48名正常健康志愿者血清样本的糖蛋白表达谱;利用聚类分析(HCA)以及Ratio值分析对肺癌患者和正常健康志愿者血清的糖蛋白糖链谱进行比较分析;结合凝集素印记技术反向验证凝集素芯片结果。再使用ROC曲线初步评估筛选出的差异凝集素所识别的糖链结构在肺癌早期诊断中潜在的应用价值。结果RCA120识别的β-Gal,Galβ-1,4GlcNAc糖链结构在所有肺癌患者中的相对丰度均显著低于正常健康志愿者;ROC曲线显示,RCA120在区分早期肺癌患者与正常健康志愿者时,AUC=0.802,P=0.004;提示血清中凝集素RCA120识别的β-Gal,Galβ-1,4GlcNAc糖链结构能够有效区分早期肺癌患者与正常健康志愿者。结论凝集素RCA120识别的β-Gal,Galβ-1,4GlcNAc糖链结构可能有助于肺癌的早期诊断。Objective Through lectin microarray,the diff erences of serum glycoprotein sugar chain spectrum between patients with stages I,II,III and IV lung cancer and healthy volunteers were analyzed,and the sugar chain structure markers related to the occurrence and development of lung cancer were found,so as to provide clinical basis for earlier discovery and identifi cation of lung cancer.Methods Lectin microarray was performed to analyze the glycoprotein expression profi les of serum samples from 54 patients with diff erent stages of lung cancer(9 patients at stages I and II,24 patients at stage III and 21 patients at stage IV)and 48 healthy volunteers in the Second Hospital Affi liated to South China University from September 2017 to October 2018.Hierarchical cluster analysis(HCA)and ratio value analysis were used to analyze the glycoprotein glycochain profiles of serum from patients with lung cancer and healthy volunteers.The results of lectin microarray were verifi ed by lectin imprinting technique reversely.The ROC curve was used to preliminarily evaluate the potential value to apply the screened sugar chain structure identifi ed by the diff erential lectin in the early diagnosis of lung cancer.Results The relative abundance ofβ-Gal and Galβ-1,4GlcNAc sugar chain structures identifi ed by RCA120 in all lung cancer patients was signifi cantly lower than that in healthy volunteers.ROC curve showed that RCA120 could distinguishpatients with early lung cancer from healthy volunteersat AUC=0.802,P=0.004,which indicated that in serumβ-Gal and Galβ-1,4GlcNAc sugar chain structuresrecognized the lectin RCA120 could eff ectively distinguish patients with early lung cancer from healthy volunteers.Conclusionβ-Gal and Galβ-1,4GlcNAc sugar chain structures recognized by lectin RCA120 may contribute to the early diagnosis of lung cancer.
