摘　　要：目的了解年龄、性别因素对应用三阶段法直立倾斜试验(HUTT)诊断血管迷走性晕厥(VVS)的影响。方法回顾性纳入2016年10月至2017年4月1162例疑诊VVS患者,将患者按照年龄分为7组(≤10岁组,11~20岁组,21~30岁组,31~40岁组,41~50岁组,51~60岁组,61~70岁组,>70岁组);分析HUTT结果在各年龄组的差异性,以及VVS在各年龄组的性别分布。结果在纳入的1162例疑似患者中,经基础+硝酸甘油HUTT共确诊505例(43.42%),经三阶段倾斜试验共确诊648例(55.8%)。HUTT在10~20岁组疑似患者中阳性率最高(60.8%),各年龄组间HUTT总阳性率差异无统计学意义(P>0.05)。总人群中女性患者的阳性率高于男性(P<0.05),但该差异主要在31~50岁年龄组中差异显著(P<0.05)。结论三阶段法HUTT较传统的基础+硝酸甘油HUTT诊断VVS的检出率有所提高。VVS在各年龄段的发生率无显著差异。女性较男性更为多发,但主要表现在31~50岁之间的晕厥患者中。Objective To investigate the effect of age and gender on the diagnosis of vasovagal syncope(VVS)with three-phase head-up tilt test(HUTT).Methods Totally 1162 consecutive patients underwent three-phase HUTT due to suspected VVS from October 2016 to April 2017 were enrolled in this study.Subjects were divided into seven groups by age(≤10 years,11~20 years,21~30 years,31~40 years,41~50 years,51~60 years,61~70 years,>70 years).Positive HUTT rate across all age groups and gender distribution in each age group were analyzed.Results Among all 1162 subjects with suspected VVS,505 subjects(43.42%)recorded a positive result by basic+nitroglycerin HUTT,and 648 subjects(55.8%)by three-phase HUTT.Positive rate was highest in the 10~20 years old group,accounting for 60.8%of all cases;There was no significant difference among all age groups.Among all subjects,female subjects exhibited a higher positive rate for three phase HUTT than their male counterparts(P<0.05),however,this gender difference was only significant in the 31~50 years old group(P<0.05).Conclusion The three-stage method HUTT has improved the detection of VVS over the conventional basic+nitroglycerin HUTT and reduced the number of missed diagnoses.The incidence of VVS among different age groups was not significantly different.Females have a higher positive rate of HUTT than males,but the significance only existed in the 31~50 years old group.
