摘　　要：目的 探讨经桡动脉途径行外周血管介入的可行性及其并发症情况。方法 回顾南阳市第一人民医院于2018年10月至2020年7月经桡动脉途径行外周血管介入手术的103例患者临床资料,共192例次手术,其中肝癌化疗栓塞术、骨肿瘤术前栓塞术、妇科肿瘤化疗栓塞术、消化道血管造影与栓塞术、膀胱癌化疗栓塞术及肾动脉成形术分别为150例次、16例次、11例次、11例次、3例次及1例次。分析患者的桡动脉穿刺、后续操作手术成功情况,并统计其相关并发症与患者接受度。结果 192例次桡动脉穿刺成功148例次,综合穿刺成功率为98.67%;经桡动途径手术成功185例次,综合手术成功例次为98.39%。其中发现相关并发症5例,包括2例次皮下血肿或淤血,1例次动脉波动减弱,1例次桡动脉痉挛,1例次前臂疼痛;42例患者曾接受过经股动脉操作患者中有40例更倾向经桡动脉途径。结论 经桡动脉途径行外周血管介入是一种安全且有效的方案,更容易被患者所接受,故值得临床推广应用。Objective To investigate the feasibility and complications of peripheral vascular intervention via radial artery.Methods The clinical data of 103 patients undergoing peripheral vascular intervention from October 2018 to July 2020 in our hospital were retrospectively reviewed.There were 192 operations.Liver cancer chemoembolization, preoperative embolization of bone tumors, chemoembolization of gynecological tumors, gastrointestinal angiography and embolization, bladder cancer chemoembolization and renal artery angioplasty were performed in 150 cases and 16 cases respectively, 11 cases, 11 cases, 3 cases and 1 case.The success of radial artery puncture and follow-up operation were analyzed, and the related complications and patient acceptance were counted.Results Totally 192 times of radial artery puncture were successful in 148 times, and the comprehensive success rate was 98.67%;The transradial approach was successful in 185 cases, and the comprehensive operation was successful in 98.39%.Related complications were found in 5 cases, including 2 times of subcutaneous hematoma or congestion, 1 time of arterial fluctuation weakening, 1 time of radial artery spasm and 1 time of forearm pain;Of the 42 patients who had undergone femoral artery operation, 40 preferred radial artery approach.Conclusion Peripheral vascular intervention via radial artery is a safe and effective scheme, which is easier to be accepted by patients, so it is worthy of clinical application.
