摘　　要：目的:探讨sCD25升高对成人肿瘤相关噬血细胞综合征(HPS)的提示意义。方法:回顾性分析2019年1月1日—2020年1月1日首都医科大学附属北京友谊医院血液科诊治的228例成人HPS患者的临床资料。结果:在诊断HPS时,肿瘤相关HPS患者的sCD25明显高于非肿瘤相关HPS患者(P<0.001);肿瘤相关HPS患者的sCD25明显高于感染相关HPS患者(P=0.001);肿瘤相关HPS患者的sCD25明显高于风湿免疫相关HPS患者(P<0.001);肿瘤相关HPS患者的sCD25明显高于不明原因相关HPS患者(P<0.001)。当sCD25水平高于23 000 ng/mL时,诊断为肿瘤相关HPS的灵敏度为70.7%,特异度为66.8%。结论:sCD25升高对成人肿瘤相关HPS具有重要的提示意义,当sCD25明显升高时,需要警惕肿瘤相关HPS的可能。Objective:To investigate the significance of elevated sCD25 in adult tumor associated hemophagocytic syndrome(HPS).Methods:The clinical data of 228 adult HPS patients treated in the Department of Hematology,Beijing Friendship Hospital,Capital Medical University from January 1,2019 to January 1,2020 were analyzed retrospectively.Results:When diagnosing HPS,sCD25 in tumor related HPS group was significantly higher than that in non-tumor related HPS group(P<0.001);sCD25 in tumor related HPS group was significantly higher than that in infection related HPS group(P=0.001);sCD25 in tumor related HPS group was significantly higher than that in rheumatic immune related HPS group(P<0.001);sCD25 in tumor related HPS group was significantly higher than that in unexplained related HPS group(P<0.001).When the level of sCD25 was higher than 23 000 ng/mL,the sensitivity for the diagnosis of tumor related HPS was 70.7% and the specificity was 66.8%.Conclusion:Elevated sCD25 has an important suggestive significance for adult tumor related HPS.When sCD25 is significantly increased,it is necessary to be alert to the possibility of tumor related HPS.
