苦参碱对胃癌化疗后不良反应及炎症标志物PLA2、SAA的影响

摘　　要：目的:探讨苦参碱对胃癌化疗后不良反应及炎症标志物磷脂酶A2(phospholipase A2,PLA2)、血清淀粉样蛋白A(serum amyloid A,SAA)的影响。方法:选择Ⅱ~Ⅲ期胃癌术后化疗患者126例,随机分为观察组(70例)和对照组(56例)。观察组采用苦参碱+FOLFOX化疗方案,对照组采用单纯FOLFOX化疗方案。观察两组化疗后不良反应发生率,血清炎症标志物PLA2、SAA水平及肿瘤标志物CEA,CA199,CA724水平的变化。结果:观察组在化疗开始后,恶心、脱发、发热、口腔溃疡,静脉炎等不良反应发生率明显低于对照组(P<0.05),血清中PLA2、SAA、CEA、CA199、CA724水平明显低于对照组(P<0.05)。结论:苦参碱可以显著降低炎症标志物水平,减少全身炎症综合症的发生,对化疗药物具有协同增敏作用。Objective:To survey the effects of matrine on adverse reaction and PLA2 and SAA after chemotherapy for gastric cancer.Methods:All 126 patients undergoing chemotherapy after the surgery of gastric cancer at stageⅡtoⅢwere chosen,randomized into the observation group(70 cases)and the control group(56 cases).The observation group adopted matrine+FOLFOX chemotherapy regimen,and the control group used FOLFOX chemotherapy regimen only.To observe the incidence of adverse reaction after the chemotherapy,the levels of serum inflammatory markers including PLA2,SAA,the levels of tumor markers CEA,CA199 and CA724.Results:In the observation group after the chemotherapy,the incidences of adverse reactions including nausea,hair loss,fever,oral ulcer,phlebitis were lower than these of the control group(P<0.05),the levels of PLA2,SAA,CEA,CA199 and CA724 were lower than these of the control group(P<0.05).Conclusion:Matrine could notably decrease the levels of inflammatory markers,reduce the incidence of systemic inflammatory syndrome,and it has synergistic sensitizing effect on chemotherapy drugs.
