CT对肾旁前间隙内原发性EGIST和十二指肠源性GIST的诊断价值

摘　　要：目的探讨CT对鉴别肾旁前间隙内原发性胃肠道外间质瘤(extra-gastrointestinal stromal tumors,EGIST)和十二指肠源性GIST的鉴别诊断价值。方法选取经手术病理证实的11例肾旁前间隙内原发性EGIST和肾旁前间隙内的10例十二指肠源性GIST患者的CT资料,重点观察肿块的大小、形态、密度和强化等情况,以及肿块与十二指肠的关系等,二者之间相互对比。结果肾旁前间隙原发性EGIST组和十二指肠源性GIST组病例之间,患者的年龄、肿块的最大横径、肿块强化程度和特点等差异无统计学意义;肿块“尖角征”的显示、肿块与十二指肠分界清晰情况,二者的差异有统计学意义(P<0.05)。结论根据肾旁前间隙内肿块的CT特点,尤其是肿块“尖角征”的显示、肿块与邻近十二指肠分界情况,CT有助于肾旁前间隙内原发性EGIST和十二指肠GIST的鉴别诊断。Objective To evaluate the value of CT in the differential diagnosis of primary extra-gastrointestinal stromal tumors(EGIST)and duodenal GIST located in the anterior pararenal space.Methods The CT manifestations of the primary EGIST(n=11)and duodenal GIST(n=10)located in the anterior pararenal space identified by surgery and pathology were retrospectively reviewed and analyzed.Emphasis was placed as follows:the CT features of the masses,especially the relationship between the masses and the duodenum.Results Between the primary EGIST and duodenal GIST,the difference of patient's age,maximum transverse diameter of mass,enhanced degree and features of the mass was not significance(P>0.05).The difference of cuspital horn sign,the relationship between the mass and duodenum in the primary EGIST and duodenal GIST was significance(P<0.05).Conclusion The CT manifestations of the mass itself,especially the cuspital horn sign,relationship between the mass and duodenum,are very helpful to make CT differential diagnosis diagnosis for the primary EGIST and duodenal GIST located in the anterior pararenal space.
