摘　　要：偏头痛是神经内科常见疾病,其临床特征是反复发作的严重的单侧搏动性头痛并伴有各种神经系统症状。根据2018年最新国际头痛分类指南标准,偏头痛分为六大亚型:无先兆偏头痛(Migraine without aura,MwoA)、先兆偏头痛(Migraine with aura,MA)及慢性偏头痛(Chronic migraine,CM)等。在偏头痛患者中,女性的发病率明显高于男性,为2~4倍,而且头痛发作更严重、更频繁、更持久,更容易向慢性偏头痛转变。目前对于偏头痛性别差异方面的研究较少,这种差异背后的发病机制尚不清楚。本文着重回顾了偏头痛的性别差异在临床表现、病理生理学、基因、神经影像学等方面的既往研究,以了解这些差异,为后续研究偏头痛发病机制及研发更有效的针对性的治疗方案提供帮助。Migraine is a common disease in neurology,which is characterized by repeated severe unilateral pulsatile headache accompanied by various neurological symptoms.According to the guidelines of International Headache Society,migraine can be divided into two subtypes:migraine without aura(MwoA)and migraine with aura(MA).Among migraine patients,the incidence rate of women is significantly higher than that of men,about 2-4 times,and their headache attacks are more serious,more frequent and longer,and they are more likely to change to chronic migraine.There are few studies on migraine based on gender differences,and the pathogenesis behind this female advantage is still unclear.This paper focuses on reviewing the gender differences in clinical manifestations,pathophysiology,genes,neuroimaging and other aspects of migraine.Understanding these differences will help to further study the pathogenesis of migraine and develop more to effective targeted treatment schemes.
