摘　　要：目的探讨角膜瓣偏离对飞秒激光辅助准分子激光角膜原位磨镶术(FS-LASIK)疗效的影响。方法回顾性病例研究。分析2017年1月至2020年12月于我院行FS-LASIK治疗近视患者共125例(250只眼)临床资料,根据角膜瓣有无偏离瞳孔中心分组,比较两组基线临床资料、视力、散光情况、角膜曲率、球镜角度及高阶像差。结果两组基线临床资料比较差异无统计学意义(P>0.05);未偏离组术后1周散光度数≤-0.5 D比例显著高于偏离组(P<0.05);两组术后4周散光度数情况比较差异无统计学意义(P>0.05);两组术后1周和4周角膜曲率和球镜角度比较差异无统计学意义(P>0.05);两组术后1周和4周球差、彗差水平比较差异无统计学意义(P<0.05);未偏离组术后1周和4周三叶草差水平均显著低于偏离组(P<0.05)。结论角膜瓣偏离对FS-LASIK术后远期视觉质量无明显影响,但可能造成早期视觉体验不佳。Objective To investigate the influence of corneal flap deviation on clinical effects after femtosecond laser-assisted laser in situ keratomileusis(FS-LASIK).Methods In this retrospective study,250 eyes of 125 patients with myopia who received FS-LASIK and 80 healthy people were collected from January 2017 to December 2020.All patients were grouped according to whether the corneal flap deviates from the pupil center or not.The difference of the baseline clinical data,visual acuity,astigmatism,corneal curvature,spherical angle and higher-order aberrations were compared between two groups.Results There was no statistical difference in baseline clinical data between two groups(P>0.05).The proportion of astigmatism≤-0.5 D in the non deviation group was significantly higher than deviation group(P<0.05).There was no significant difference in astigmatism at 4 weeks after operation between two groups(P>0.05).The corneal curvature and spherical angle changes at 1 week and 4 weeks after operation between two groups were not statistically different(P>0.05).The water content of clover difference at 1 week and 4 weeks after operation was significantly lower in the non deviation group as compared with the deviation group(P<0.05).Conclusions Corneal flap deviation has no significant effect on long-term visual quality after FS-LASIK,but it may cause poor early visual experience.
