下斜肌分级前转位术治疗下斜肌亢进的临床观察

摘　　要：目的观察下斜肌分级前转位术与下斜肌切除术对治疗下斜肌亢进的临床疗效。方法回顾性病例研究。抽取2018年10月至2020年2月我院收治的64例(111只眼)下斜肌亢进患者,其中观察组32例(56只眼)行下斜肌分级前转位术,对照组32例(55只眼)采用下斜肌切除术,术后6个月,对患者下斜肌亢进的残留程度和第一眼位垂直斜视残留程度进行对比。结果术后下斜肌亢进为0的患者比例观察组要高于对照组;术后下斜肌出现过度减弱的患者比例观察组要低于对照组;术后第一眼位残留斜视度≤4^(Δ)的患者比例,观察组要高于对照组。结论下斜肌分级前转位术在改善不同强度的下斜肌亢进方面对比传统下斜肌切除术更有可预见性和术后稳定性,同时在改善垂直斜视方面也是分级前转位术优于下斜肌切除术。Objective To observe the clinical effect of graded anterior transposition of inferior oblique in the treatment of inferior oblique overaction.Methods Totally 64 patientes(111 eyes)with inferior oblique overaction who were admitted to the hospital from October 2018 to February 2020 were selected.They were divided into the observation group(n=32,56 eyes)and the contrast group(n=32,55 eyes).The observation group was treated with graded anterior transposition of inferior oblique muscle.The contrast group was treated with inferior oblique muscle resection.We compared with the residual degree of inferior oblique overaction and the residual degree of first eye vertical strabismus in 6 months after surgery.Results The proportion of patients with 0 postoperative inferior oblique overaction in the observation group was higher than that in the control group(P<0.05);The proportion of patients with excessive weakening of inferior oblique muscle after operation in the observation group was lower than that in the control group(P<0.05).The proportion of postoperative patients with residual strabismus≤4^(Δ)in the observation group was higher than that in the control group(P<0.05).Conclusions Compared with traditional inferior oblique resection,graded anterior transposition of inferior oblique is more predictable and stable.In addition,in improving vertical strabismus,the cure rate of graded anterior transposition is better than that of inferior oblique resection.
