摘 要:目的探讨青少年近视眼患者长期配戴角膜塑形镜后中央角膜厚度、角膜内皮细胞的影响。方法回顾性病例研究。收集2018年1月至2020年1月本院门诊就诊45例(90只眼)中低度近视青少年并配戴角膜塑形镜者为研究对象,纳入研究组。同时收集同时期45例(90只眼)中低度近视使用框架眼镜者为对照组。比较两组患者在配戴框架眼镜及角膜塑形镜后3个月、6个月、12个月后的屈光力增长的情况,检测并分析研究组配戴角膜塑形镜后在3个月、6个月、12个月后的中央角膜厚度、角膜内皮细胞密度的变化,计量质量采用卡方检验,计数资料采用t检验,以P<0.05为有统计学意义。结果两组患者在年龄、性别、戴镜前视力、屈光度的基线水平比较上,均P>0.05,差异无统计学意义。两组患者在戴镜后3个月时屈光度增长的比较上P>0.05,差异无统计学意义。两组患者在使用框架眼镜及角膜塑形镜后6个月、12个月的屈光度增长上,两组结果比较均P<0.05,差异有统计学意义。对配戴3个月、6个月、12个月角膜内皮及中央角膜厚度检测上,均P>0.05,差异无统计学意义。结论配戴角膜塑形镜对中央角膜厚度和角膜内皮细胞不会产生显著影响,是安全、有效的控制青少年中低度近视的方法。Objective To investigate the effects of orthokeratology on central corneal thickness and corneal endothelial cells in adolescents with moderate and low myopia.Methods Retrospective case study.A total of 45 cases(90 eyes)of adolescents with moderate and low myopia wearing orthokeratology lens were collected from our out-patient clinic from January 2018 to January 2020,and included in the study group.A total of 45 cases(90 eyes)with moderate and low myopia using glasses were collected as the control group.Comparing a increased trend of refractive power between the two groups in the use of frame glasses and corneal shape at 3 months,6 months and 12 months.The differences of central corneal thickness and corneal endothelial cell density in the study group at 3 months,6 months and 12 months were tested by the chi-squared test.Results There was no significantly difference(P>0.05)in terms of age,gender,visual acuity before wearing glasses and baseline level of refraction between the two group.Comparison of the refraction increase between the two groups at 3 months after wearing the lens was not statistically different(P>0.05).The results of the increase of diopter at 6 months and 12 months after the use of glasses and orthokeratology lens between the two groups were statistically different(P<0.05).The results of corneal endothelium and the central corneal thickness test at 3 months,6 months and 12 months were not statistically different(P>0.05).Conclusions Wearing orthokeratology lens had no significant effect on central corneal thickness and corneal endothelial cells.It is a safe and effective method to control middle and low myopia in teenagers.
