摘　　要：目的探讨血清癌胚抗原(CEA)、胃泌素释放肽前体(ProGRP)、细胞角蛋白19(CYFRA21-1)、神经元特异性烯醇化酶(NSE)在肺癌鉴别诊断中的临床价值。方法选取经病理学检查确诊的肺癌患者80例(肺癌组)、肺部良性疾病患者40例(良性组),检测患者血清CEA、ProGRP、NSE、CYFRA21-1蛋白水平,以病理学诊断结果作为参考计算各项指标鉴别诊断肺癌的效能。结果肺癌组患者的血清CEA、ProGRP、NSE、CYFRA21-1水平显著高于良性组,差异均具有统计学意义(P<0.05)。血清CEA、ProGRP、NSE、CYFRA21-1联合应用鉴别诊断肺癌的灵敏度、特异度、漏诊率和误诊率分别为87.00%、80.00%、12.50%和20.00%。结论CEA、ProGRP、NSE、CYFRA21-1蛋白水平联合检测对于鉴别诊断肺癌具有一定的临床意义。Objective To explore the clinical value of serum carcinoembryonic antigen(CEA),gastrin-releasing peptide precursor(ProGRP),cytokeratin 19(CYFRA21-1)and neuron-specific enolase(NSE)in differential diagnosis of lung cancer.Methods The serum levels of CEA,ProGRP,NSE and CYFRA21-1 protein were measured in 80 patients with lung cancer and 40 patients with benign lung diseases.The pathological indexes were taken as the reference to calculate the diagnostic indexes for differential diagnosis of lung cancer.Results The levels of serum CEA,ProGRP,NSE and CYFRA21-1 in lung cancer group were significantly higher than those in benign group(P<0.05).The sensitivity,specificity,missed diagnosis rate and misdiagnosis rate of combined application of serum CEA,ProGRP,NSE and CYFRA21-1 in differential diagnosis of lung cancer were 87.00%,80.00%,12.50%and 20.00%,respectively.Conclusion The combined detection of CEA,ProGRP,NSE and CYFRA21-1 protein levels has certain clinical significance in differential diagnosis of lung cancer.
