乙肝肝硬化并发原发性肝癌的相关危险因素分析

摘　　要：目的分析乙肝肝硬化并发原发性肝癌的危险因素。方法选取乙肝肝硬化并发原发性肝癌患者61例作为发生组,并以1∶1配比选取同期乙肝肝硬化无原发性肝癌患者61例作为未发生组,采用我院自制调查问卷收集两组性别、年龄、有无原发性肝癌遗传史、有无糖尿病、有无酒精性脂肪肝等信息,分析乙肝肝硬化并发原发性肝癌的影响因素。结果有无原发性肝癌遗传史、有无糖尿病、有无酒精性脂肪肝、烟草依赖程度、酒精依赖程度、HBV-DNA载量和有无抗病毒治疗是乙肝肝硬化并发原发性肝癌的影响因素(P<0.05);logistic回归分析显示,原发性肝癌遗传史、糖尿病、酒精性脂肪肝、烟草重度依赖、酒精重度依赖、HBV-DNA载量>10^(4)是乙肝肝硬化并发原发性肝癌的危险因素,抗病毒治疗是乙肝肝硬化并发原发性肝癌的保护因素(P<0.05)。结论乙肝肝硬化并发原发性肝癌影响因素较多,临床应依据上述因素积极实施抗病毒治疗等干预措施,以降低原发性肝癌发生风险,改善预后。Objective To analyze the risk factors of liver cirrhosis complicated with primary liver cancer.Methods 61 patients with liver cirrhosis complicated with primary liver cancer were selected as occurrence group,In addition,61 patients with liver cirrhosis without primary liver cancer were selected as non-occurrence group with 1:1 ratio,Using our self-made questionnaire to collect information of sex,age,genetic history of primary liver cancer,diabetes and alcoholic fatty liver,To analyze the influencing factors of hepatitis B cirrhosis complicated with primary liver cancer.Results The genetic history of primary liver cancer,diabetes,alcoholic fatty liver,tobacco dependence,alcohol dependence,HBV-DNA load and antiviral therapy were the influencing factors(P<0.05);logistic regression analysis showed,the genetic history of primary liver cancer,diabetes mellitus,alcoholic fatty liver disease,tobacco heavy dependence,alcohol heavy dependence,HBV-DNA load>10^(4) were risk factors for hepatitis b cirrhosis complicated with primary liver cancer,Antiviral therapy was a protective factor for hepatitis B cirrhosis complicated with primary liver cancer(P<0.05).Conclusion Hepatitis B cirrhosis complicated with primary liver cancer has many influencing factors.According to the above factors,antiviral therapy should be actively implemented in order to reduce the risk of primary liver cancer and improve the prognosis.
