摘 要:目的 比较人牙周膜细胞接种于不同牙周治疗方法处理的根面粘附、增殖情况,初步探讨Er:YAG激光用于牙周非手术治疗时的合适治疗参数。方法 以新鲜拔除的重度牙周炎患牙为研究对象,分别采用单纯超声刮治,超声联合手工刮治以及超声联合手工刮治辅以不同参数Er:YAG激光照射处理患牙根面,并将体外培养的人牙周膜成纤维细胞接种于根面上。采用倒置显微镜、扫描电镜、CCK-8法等观察方法,评价人牙周膜成纤维细胞在根面的粘附、增殖情况。结果 不同牙周治疗方法处理后各组根片上均可看到人牙周膜细胞附着。人牙周膜细胞的粘附和增殖能力受牙根表面形态的影响。研究结果表明,与单纯超声刮治相比,超声联合手工刮治以及超声联合手工刮治辅以40mJ/P、60mJ/P的Er:YAG激光更有利于细胞的粘附与增殖。结论 超声联合手工刮治辅以40mJ/P的Er:YAG激光组更有利于人牙周膜细胞的粘附与增殖,有望成为传统牙周非手术治疗的辅助手段之一,但尚需要长期的基础和临床研究予以证实。Objective To compare the adhesion and proliferation of human periodontal membrane cells inoculated on the root surface of different periodontal treatment methods, and to preliminarily explore the appropriate treatment parameters of Er:YAG laser for periodontal nonsurgical treatment. Methods Taking freshly extracted teeth with severe periodontitis as the research object, the root surfaces of the teeth were treated with simple ultrasonic scaling, ultrasonic combined with manual scaling and ultrasonic combined with manual scaling combined with Er:YAG laser with different parameters, and inoculated human periodontal ligament fibroblasts cultured in vitro.Inverted microscope, scanning electron microscope, and CCK-8 method were used to evaluate the adhesion and proliferation of human periodontal ligament fibroblasts on the root surface. Results The attachment of human periodontal ligament cells can be seen on the root slices of each group after different periodontal treatment methods. The adhesion and proliferation of human periodontal ligament cells are affected by the morphology of the tooth root surface. The results showed that compared with simple ultrasonic scaling, ultrasonic combined with manual scaling and ultrasonic combined with manual scaling combined with 40mJ/P and 60mJ/P Er: YAG laser were more conducive to cell adhesion and proliferation. Conclusion Ultrasonic combined with manual scaling combined with 40mJ/P Er:YAG laser group is more conducive to the adhesion and proliferation of human periodontal ligament cells, which is expected to become one of the auxiliary means of traditional periodontal nonsurgical treatment.
