综合护理干预对老年重症肺炎患者身心的影响

摘　　要：目的研究护理干预对老年重症肺炎合并呼吸机相关肺炎患者预后的影响及其抑郁、焦虑情绪的影响。方法根据随机数字表法将2018年2月~2021年2月期间本院接收的老年重症肺炎合并呼吸机相关肺炎患者76例分为两组,常规药物治疗的基础上,在对照组38例进行常规护理,观察组38例实施综合护理干预。将两组的焦虑抑郁评分、血气分析指标、肺功能指标、并发症发生情况、呼吸机使用时间、住院时间、护理满意度进行比对。结果观察组患者干预后的焦虑自评量表(Self-Rating Anxiety Scale,SAS)评分、抑郁自评量表(Self-rating depression scale,SDS)评分、二氧化碳分压(PaCO_(2))、内源性呼吸末正压(PEEPi)低于对照组,干预后的氧分压(PaO_(2))、血氧饱和度(SaPO_(2))、最大呼吸流量(PEF)均高于对照组(P<0.05);观察组患者的并发症发生率低于对照组,呼吸机使用时间、住院时间短于对照组,护理满意度高于对照组(P<0.05)。结论对老年重症肺炎合并呼吸机相关肺炎患者实施综合护理干预。有助于缓解患者的负面情绪,较好地改善血气分析指标和肺功能。Objective To study the effect of comprehensive nursing intervention on the prognosis and depression and anxiety of elderly patients with severe pneumonia complicated with ventilator-associated pneumonia.Methods According to the random number table method,76 elderly patients with severe pneumonia complicated with ventilator-associated pneumonia received in our hospital from February 2018 to February 2021 were divided into two groups.38 cases in the control group received routine nursing and 38 cases in the observation group received comprehensive nursing intervention.The anxiety and depression scores,blood gas analysis indexes,pulmonary function indexes,complications,ventilator use time,hospital stay and nursing satisfaction of the two groups were compared.Results Self-assessment scale of anxiety in the observation group(Self-Rating Anxiety Scale,SAS)score,depression Self-Evaluation Scale(Self-rating depression scale,SDS)score,carbon dioxide partial blood pressure(PaCO_(2)),and endogenous final respiratory positive blood pressure(PEEPi)were lower than those in the control group,Oxygen partial pressure(PaO_(2)),blood oxygen saturation(SaPO_(2)),and maximum respiratory flow(PEF)after the intervention were all higher than those in the control group(P<0.05).The incidence of complications in the observed group was lower than in the control group,The ventilator duration and hospital stay were shorter than those in the control group,Care satisfaction was higher than that in the control group(P<0.05).Conclusion Comprehensive nursing intervention for elderly patients with severe pneumonia complicated with ventilator-associated pneumonia is helpful to alleviate their negative emotions,better improve blood gas analysis indexes and lung function,and obtain a good prognosis.
