摘　　要：目的:评估伴有中高度散光的近视患者配戴Toric角膜塑形镜后的主观视觉质量,探讨其与角膜高阶像差的相关性。方法:随机选取36例Toric角膜塑形术后的近视患者,采用NEI-VFQ-25量表和主观视觉症状问卷对纳入患者的主观视觉质量进行评定,并应用Pentacam眼前节分析系统测量36例(72眼)的角膜高阶像差,分别记录3~6阶的22项Zernike系数。应用SPSS19.0统计软件进行数据的K-S正态性检验,用Pearson或Spearman相关法分析NEI-VFQ-25量表和主观视觉症状问卷的各维度与各项Zernike系数的相关性。结果:塑形术后的眩光评分与慧差(Z7、Z8)、闪光评分与慧差(Z7、Z8)和初级球差(Z12)、晕环评分与慧差(Z7、Z8)和二级散光(Z11、Z13、)、夜视觉障碍评分与慧差(Z7、Z8)之间均存在显著的正相关(P<0.05),其余各主观视觉质量评分与各项高阶像差无显著性相关关系。结论:Toric角膜塑形术后患者的主观视觉质量与角膜高阶像差有相关性,异常的视觉症状提示了初级球差、慧差、二级散光的增大。Objective:To evaluate the subjective visual quality of myopic patients with astigmatism after Toric orthokeratology,and to study its correlations with corneal higher order aberrations(HOA).Methods:36 cases of myopia patients after Toric orthokeratology were randomly selected.The NEI-VFQ-25 scale and the subjective visual symptom questionnaire were used to evaluate the subjective visual quality of the included patients,and the Pentacam anterior segment analysis system was used to measure 36 cases(72 eyes)corneal higher-order aberrations,and 22 Zernike coefficients of orders 3-6 were recorded.SPSS 19.0 statistical software was used to perform K-S normality test of the data,and the correlation between each dimension of NEI-VFQ-25 scale and subjective visual symptom questionnaire and various Zernike coefficients was analyzed by Pearson or Spearman correlation method.Results:There was a significant positive correlation between glare score and coma(Z7,Z8),flash score and coma(Z7,Z8),primary spherical aberration(Z12),halo score and coma(Z7,Z8)and secondary astigmatism(Z11,Z13),night vision impairment score and coma(Z7,Z8)(P<0.05).There was no significant correlation between other subjective visual quality scores and higher-order aberrations.Conclusion:Subjective visual quality of patients after Toric orthokeratology is correlated with corneal HOA.Abnormal visual symptoms indicate increase of primary spherical aberration,coma and secondary astigmatism.
