摘　　要：目的探讨Marjolin溃疡的临床病理特点。方法回顾分析该院2016-2020年收治的96例Marjolin溃疡和113例非Marjolin溃疡患者的临床和病理资料。结果Marjolin溃疡男性患者居多(57.3%),年龄主要分布在40~70岁,发病原因以烧伤为主(75.0%),病程20年以上者占74.0%,发病部位在下肢占47.9%,临床表现为皮肤表面溃疡,溃疡面积多为2~5 cm,恶变肿瘤类型为高分化鳞状细胞癌占42.7%,浸润至真皮层占79.2%。患者随访4个月到5年,5例患者死亡。结论Marjolin溃疡主要发生在中老年男性患者,发病部位以下肢为主,病程较长,多恶变为高分化鳞状细胞癌,侵及真皮层,预后较非Marjolin溃疡差。Objective To investigate the clinicopathological characteristics of Marjolin ulcer.Methods A retrospective study was performed on 209 cases of Marjolin ulcer and non-Marjolin ulcer treated in this hospital during 2016-2020.Results Marjolin ulcer was dominated by the male patients,accounting for 57.3%,the main age was distributed in 40-70 years old,the cause of the disease was burn(75.0%),the disease course of more than 20 years accounted for 74%,the affected site was in the lower extremity,accounting for 47.9%,and the clinical manifestations were the skin surface ulcers,the ulcer area was usually 2-5 cm,the type of malignant tumor was well-differentiated squamous cell carcinoma(42.7%),and infiltrating into the dermis accounted for 79.2%.The patients were followed up from 4 months to 5 years,and 5 cases died.Conclusion Marjolin ulcer mainly occurs in the middle-aged and elderly male patients.The disease site is the lower extremity.The course of the disease is longer.Its malignant transformation is mostly well-differentiated squamous cell carcinoma,which invades the dermis.The prognosis is worse than that of non-Marjolin ulcers.
