近端胃切除与全胃切除Roux-en-Y方式重建消化道治疗胃底贲门癌的效果及生活质量的对比分析

摘　　要：目的:对比近端胃切除与全胃切除Roux-en-Y方式重建消化道治疗胃底贲门癌的效果及生活质量。方法:随机抽取60例胃底贲门癌患者作为此次研究对象,病例起止时间为2015年1月至2018年6月,60例患者分组方式为随机数字表法,即观察组30例实施全胃切除Rouxen-Y方式重建消化道,对照组30例实施近端胃切除,对比两种方法的临床效果。结果:(1)观察组患者治疗后的1个月、6个月、12个月、18个月以及24个月生活质量评分高于对照组患者,对比组间数据如P<0.05,具有统计学意义;(2)观察组患者治疗后并发症发生率为3.33%,对照组患者治疗后并发症发生率为20%,对比组间数据如P<0.05,具有统计学意义。结论:全胃切除Roux-en-Y方式重建消化道治疗胃底贲门癌可提升患者的生活质量,降低并发症发生率。Objective:To compare the effect and quality of life between proximal gastrectomy and total gastrectomy with Roux-en-Y reconstruction in the treatment of gastric cardia cancer.Methods:60 patients with gastric cardia cancer were randomly selected as the research object.The starting and ending time of the cases was from January 2015 to June 2018.The grouping method of 60 patients was random number table method,that is,30 cases in the observation group received total gastrectomy,Roux-en-Y reconstruction of digestive tract,and 30 cases of proximal gastrectomy in the control group.The clinical effects of the two methods were compared.Results:(1)The quality of life scores of patients in the observation group at 1 month,6 months,12 months,18 months and 24 months after treatment were higher than those in the control group,and the data between the two groups were P<0.05,which was statistically significant;(2)the incidence of complications in the observation group was 3.33%,and that in the control group was 20%,with statistical significance.Conclusion:Total gastrectomy with Rouxen-Y reconstruction of digestive tract can improve the quality of life and reduce the incidence of complications.
