摘　　要：目的探讨动态监测体循环肿瘤细胞(circulating tumor cells,CTC)对转移性三阴型乳腺癌(triple-negative breast cancel,TNBC)化疗疗效的影响。方法回顾性分析茂名市人民医院TNBC患者82例,按照是否接受CTC检测并根据CTC结果更换化疗治疗方案分成观察组和对照组,每组41例。记录2组患者化疗有效率、总生存期(overall survival,OS)、无进展生存期(progress free survival,PFS)和卡氏功能状态量表(Karnofsky performance status,KPS)评分。结果观察组患者3个月时化疗有效率和KPS评分显著高于对照组(P<0.05),观察组患者中位PFS和中位OS均显著高于对照组(P<0.05)。结论基于动态监测CTC调整化疗方案可显著提高转移性TNBC患者化疗有效率,延缓疾病进展,延长生存时间,提高患者生活质量,值得在临床上推广。Objective To explore the effect of circulating tumor cells(CTC)on chemotherapeutic effect in triple-negative breast cancer(TNBC)patients.Methods 82 TNBC patients were analyzed and assigned to the observation group and the control group according to if received CTC detection and adjusted chemotherapy regimen on the basis of CTC results.Response rate to chemotherapy,overall survival(OS),progress free survival(PFS)and Karnofsky performance status(KPS)scores were recorded.Results Response rate to chemotherapy and KPS scores at 3 m were significantly higher in the observation group than those in the control group(P<0.05),and median PFS and OS were also significantly higher in the observation group than those in the control group(P<0.05).Conclusion Adjusting chemotherapy regimen on the basis of CTC results could increase response rate to chemotherapy,survival time and living quality and delay progression,which is worth promoting in clinical work.
