摘 要:目的探讨三细胞紧密连接蛋白(tricellulin)在结直肠癌中表达的临床意义及其与血管生成的关系。方法选择2013年3月至2013年12月广西医科大学第一附属医院结直肠肛门外科收治的21例结直肠癌患者为研究对象,收集手术切除的结直肠癌组织及其对应的癌旁正常组织(距离癌组织大于10 cm)标本各21例。通过免疫组化实验检测结直肠癌组织与癌旁正常组织中tricellulin和血管内皮生长因子(VEGF)的表达并进行微血管密度(MVD)计数,Spearman秩相关分析结直肠癌组织中tricellulin和VEGF表达的关系,探究tricellulin、VEGF的表达与MVD的关系,以及三者与患者临床病理特征的关系。结果结直肠癌组织中tricellulin阳性表达比例、VEGF阳性表达比例均高于癌旁正常组织,MVD大于癌旁正常组织(均P<0.05);结直肠癌组织中tricellulin与VEGF的表达呈正相关(r=0.79,P<0.05);tricellulin阳性表达、VEGF阳性表达的结直肠癌组织中的MVD分别大于tricellulin阴性表达、VEGF阴性表达的结直肠癌组织(均P<0.05);tricellulin、VEGF的表达与患者年龄、性别无关(均P>0.05);tricellulin、VEGF的表达与结直肠癌患者的淋巴结转移、远处转移、TNM分期有关(均P<0.05);不同性别、年龄的结直肠癌患者的MVD比较差异无统计学意义(均P>0.05);有淋巴结转移、远处转移的结直肠癌患者的MVD大于无淋巴结转移、远处转移的结直肠癌患者(均P<0.05),TNM分期为Ⅲ+Ⅳ期的结直肠癌患者的MVD大于TNM分期为Ⅰ+Ⅱ期的结直肠癌患者(P<0.05)。结论Tricellulin可能参与调节结直肠癌血管生成,影响肿瘤转移和发展。Objectives To investigate the clinical significance of tricellular tight junction protein(tricellulin)in colorectal cancer and its relationship with angiogenesis.Methods Twenty-one patients with colorectal cancer treated at the Department of Colorectal&Anal Surgery of the First Affiliated Hospital of Guangxi Medical University between March 2013 and December2013 were recruited.Twenty-one specimens of colorectal cancer and normal tissues(>10 cm from the tumor edge)were collected via surgical resection.The expression of tricellulin and vascular endothelial growth factor(VEGF)were analyzed by immunohistochemistry and microvessel density(MVD).Spearman’s correlation was used to analyze the correlation between the expression of tricellulin and VEGF.To explore the correlation between tricellulin,VEGF expression and MVD,and their relationship with clinicopathological characteristics.Results The proportion of tricellulin positive expression and VEGF positive expression in colorectal cancer tissues was higher than that of normal tissues.The MVD was higher in colorectal cancer tissues than in normal tissues(P<0.05).The expression of tricellulin was positively correlated with VEGF in colorectal cancer tissues(r=0.79,P<0.05).MVD was enhanced in tricellulin or VEGF positive tissues compared with tricellulin or VEGF negative tissues(P<0.05).The expression of tricellulin and VEGF was associated with lymph node metastasis,distant metastasis,TNM stage rather than age and gender(P<0.05).There was no significant difference in MVD among colorectal cancer patients of different genders and ages(P>0.05).The MVD of colorectal cancer patients with lymph node metastasis and distant metastasis was higher than that of colorectal cancer patients without metastasis and distant metastasis(P<0.05).The MVD of colorectal cancer patients with stageⅢand stageⅣwas higher than patients with stageⅠand stageⅡ(P<0.05).Conclusion Tricellulin may be involved in the angiogenesis of colorectal cancer and influence tumor metastasis and development.
