摘 要:目的通过对直肠癌患者肿瘤部位和癌旁部位及健康直肠黏膜菌群进行分析,探讨直肠黏膜菌群与直肠癌的关系。方法选取2018年7月至2019年9月温州医科大学附属第一医院行直肠癌根治手术的94例直肠癌患者肿瘤黏膜(CM组)和癌旁黏膜(AM组),100例健康者直肠黏膜(NM组)作为研究对象。细菌培养定量检测患者和正常黏膜主要细菌,高通量测序分析3组黏膜菌群,实时荧光定量聚合酶链反应(qPCR)法检测双歧杆菌等关键菌丰度差异。正态分布资料采用t检验,非正态分布资料采用K-W检验。结果培养显示双歧杆菌在直肠癌肿瘤黏膜高于健康者(6.60±0.81比5.66±0.68,t=10.312,P<0.01)、乳酸杆菌在直肠癌肿瘤黏膜高于健康者(6.58±0.59比5.44±0.71,t=13.527,P<0.01)、肠球菌在直肠癌肿瘤黏膜高于健康者(6.22±0.87比4.64±0.80,t=12.351,P<0.01)。16SrRNA扩增子焦磷酸测序显示直肠癌患者CM及AM菌群多样性和丰富度均显著高于NM,qPCR结果显示NM中嗜黏液艾肯曼菌、消化球菌、长双歧杆菌、两歧双歧杆菌丰度分别是CM的12.55倍(t=13.500,P<0.05)、4.89倍(t=10.960,P<0.05)、2.27倍(t=8.242,P<0.05)、1.97倍(t=5.230,P<0.05)。结论直肠癌患者直肠黏膜菌群与健康人存在显著差异,直肠癌患者的肿瘤局部黏膜菌群多样性及丰度均发生了明显变化,局部黏膜菌群改变可能在直肠癌发生发展中发挥了一定作用。Objective To reveal the potential relationship between the changes of rectal mucosal flora and the tumorigenesis of rectal cancer by analyzing the mucosal flora of patients with rectal cancer and the healthy individuals.Methods Totally,94 patients with rectal cancer undergoing surgical resection from July 2018 to September 2019 at the First Affiliated Hospital of Wenzhou Medical University and 100 healthy individuals were enrolled.Samples of rectal mucus were collected from the patients[cancerous mucus(CM)and adjacent mucus(AM)]and the healthy controls[normal mucus(NM)].Aerobic or anaerobic cultures were used to test mucosalflora and 16S rRNA sequencing was applied to analyze the characteristics of the mucosal flora.The real-time quantitive polymerase chainreaction(qPCR)was used to verify the different abundance of key bacteria between the patients and healthy groups.The student’s t test for the data of normal distribution and Kruskal-Wallis test for the data of non-normal distribtion were used to analyze the data.Results Bacterial count results showed that the number of Bifidobacterium,Lactobacillus and Enterococcus was significantly greater in CM than in NM[(6.60±0.81)vs.(5.66±0.68)lgcfu/g,t=10.312,P<0.01;(6.58±0.59)vs.(5.44±0.71)lgcfu/g,t=13.527,P<0.01;(6.22±0.87)vs.(4.64±0.80)lgcfu/g,t=12.351,P<0.01].16SrDNA sequencing showed that the diversity and abundance of the rectal mucosa flora were significantly higher in the patients group than in the healthy groups.The relative abundance of Aikenmania muciniphila,Peptococcus,Bifidobacterium longum,and Bifidobacterium bifidum in NM was 12.55(t=13.500,P<0.05),4.89(t=10.960,P<0.05),2.27(t=8.242,P<0.05),and 1.97(t=5.230,P<0.05)times of that in CM,respectively.Conclusion The floraof rectal mucosa was significantly different between patients and the healthy individuals.The changes of the flora abundance and diversity at the rectal mucosa may play an important role in the development of rectal cancer.
