曲安奈德联合丹参酮注射液治疗口腔黏膜下纤维化的临床效果

摘　　要：目的探讨曲安奈德联合丹参酮注射液治疗口腔黏膜下纤维化(OSF)的临床效果。方法选择2019年9月至2020年12月在我院口腔科进行OSF治疗的118例患者为研究对象,采用单双号随机数字法将其分为对照组和试验组,各59例。对照组患者采用曲安奈德注射液进行治疗,试验组患者在对照组基础上采用丹参酮注射液进行治疗。比较两组患者的临床疗效、黏膜病损面积、最大张口度、转化生长因子-β1(TGF-β1)、白细胞介素-6(IL-6)水平、疼痛程度、口腔健康状况、血流变学指标。结果试验组的治疗总有效率显著高于对照组(P<0.05)。治疗后4个月,两组患者的黏膜病损面积显著减小,最大张口度显著增大,且试验组优于对照组(P<0.05)。治疗后4个月,两组患者的TGF-β1、IL-6水平均显著降低,且试验组显著低于对照组(P<0.05)。治疗后4个月,两组患者的VAS、OHIP-14评分均显著降低,且试验组显著低于对照组(P<0.05)。治疗后4个月,试验组的低切全血黏度、红细胞压积及FBI水平均显著低于治疗前及对照组(P<0.05)。讨论曲安奈德联合丹参酮注射液治疗OSF的效果显著优于单用曲安奈德,其能减轻患者的疼痛程度,改善口腔健康状况,减小黏膜病损面积,增大最大张口度,改善生化指标及血流变学指标水平,值得在临床上推广。Objective To investigate the clinical effect of triamcinolone acetonide combined with tanshinone injection in the treatment of oral submucosal fibrosis(OSF).Methods A total of 118 patients received OSF treatment in the stomatology department of our hospital from September 2019 to December 2020 were selected as the research objects.The patients were divided into control group and experimental group by single and double number random number method,with 59 cases in each group.The control group were treated with triamcinolone acetonide injection,and the experimental group were treated with tanshinone injection on the basis of the control group.The clinical efficacy,mucosal lesion area,maximum mouth opening,transforming growth factor-β1(TGF-β1),interleukin-6(IL-6)levels,pain degree,oral health status and hemorheological indexes were compared between the two groups.Results The total effective rate of treatment of the experimental group was significantly higher than that of the control group(P<0.05).Four months after treatment,the mucosal lesion area of the two groups decreased significantly,and the maximum mouth opening increased significantly,and those of the experimental group were better than the control group(P<0.05).Four months after treatment,the levels of TGF-β1 and IL-6 in both groups decreased significantly,and those in the experimental group were significantly lower than the control group(P<0.05).Four months after treatment,the VAS and OHIP-14 scores of the two groups decreased significantly,and those of the experimental group were significantly lower than the control group(P<0.05).Four months after treatment,the low shear whole blood viscosity,hematocrit and FBI level in the experimental group were significantly lower than those before treatment and in the control group(P<0.05).Conclusion The effect of triamcinolone acetonide combined with tanshinone injection in the treatment of OSF is significantly better than that of triamcinolone acetonide alone.The method can reduce the pain degree of patients,improv
