小梁切除联合虹膜周切术对急性原发性闭角型青光眼患者视力、并发症的影响

摘　　要：目的探究小梁切除联合虹膜周切术对急性原发性闭角型青光眼患者视力、并发症的影响。方法选取2017年6月至2020年6月本院收治的70例急性原发性闭角型青光眼患者为研究对象,按治疗方法将其分为参照组和观察组,每组35例。参照组开展虹膜周切术治疗,观察组开展小梁切除联合虹膜周切术治疗。比较两组患者的裸眼视力、眼压、中央前房深度、周边前房深度、房角宽度、眼轴、并发症发生情况、炎性因子指标及血流动力学指标。结果术后1个月,观察组的裸眼视力高于参照组,眼压低于参照组,差异有统计学意义(P<0.05)。术后1个月,观察组的中央前房深度、房角宽度大于参照组,差异有统计学意义(P<0.05)。观察组的并发症总发生率为5.71%,低于参照组的22.86%,差异有统计学意义(P<0.05)。术后24 h,观察组的IL-6、hs-CRP、TNF-α、Hcy水平低于参照组,差异有统计学意义(P<0.05)。术后24 h,观察组的RI低于参照组,EDV、PSV高于参照组,差异有统计学意义(P<0.05)。结论小梁切除联合虹膜周切术治疗急性原发性闭角型青光眼效果显著,可改善患者视力、眼压、中央前房深度、房角宽度情况,降低并发症发生率及炎性因子指标水平,改善机体血流动力学指标,值得推广。Objective To explore the effects of trabeculectomy combined with peripheral iridectomy on vision and complications in patients with acute primary angle-closure glaucoma.Methods A total of 70 patients with acute primary angle-closure glaucoma admitted in our hospital from June 2017 to June 2020 were selected as the research objects and divided into reference group and observation group according to the treatment method,with 35 cases in each group.The reference group was treated with peripheral iridectomy,and the observation group was treated with trabeculectomy combined with peripheral iridectomy.The naked vision,intraocular pressure,central anterior chamber depth,peripheral anterior chamber depth,width of chamber angle,optic axis,occurrence of complications,inflammatory factors and hemodynamic indexes of the two groups were compared.Results One month after operation,the naked vision of the observation group was higher than that of the reference group,the intraocular pressure was lower than that of the reference group,and the differences were statistically significant(P<0.05).One month after operation,the central anterior chamber depth and width of chamber angle in the observation group were greater than those in the reference group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was 5.71%,which was lower than 22.86% in the reference group,and the difference was statistically significant(P<0.05).At 24 h after operation,the levels of IL-6,hs-CRP,TNF-α and Hcy in the observation group were lower than those in the reference group,and the differences were statistically significant(P<0.05).At 24 h after operation,the RI of the observation group was lower than that of the reference group,the EDV and PSV were higher than those of the reference group,and the differences were statistically significant(P<0.05).Conclusion Trabeculectomy combined with peripheral iridectomy in the treatment of acute primary angle-closure glaucoma has a significant effect
