经济状况对脑卒中偏瘫患者及其配偶二元应对水平的影响

摘　　要：目的调查经济状况对脑卒中偏瘫患者及其配偶二元应对水平的影响。方法采用一般资料调查表、经济状况调查表、二元应对量表对2020年10月-2021年8月619对住院脑卒中偏瘫患者及其配偶进行问卷调查。结果619对患者及配偶双方二元应对均处于低水平的例数为243例(39.3%),双方二元应对水平处于中水平的为226例(36.5%),双方二元应对水平均处于双高水平的例数为150例(24.2%)。单因素分析显示,患者年龄、患者文化程度、配偶年龄、配偶文化程度、婚龄、患者职业状况、配偶职业状况、家庭月平均收入、赡养父母、抚养子女、夫妻双方财务管理与脑卒中偏瘫患者及其配偶二元应对水平有关,差异有统计学意义(P<0.05)。多因素分析显示,家庭月平均收入水平高、财务非共管的患者二元应对水平相对较高(P<0.05);患者职业状况为在职的患者配偶二元应对水平相对较高(P<0.05)。结论脑卒中偏瘫患者及其配偶二元应对水平处于中等水平;经济状况对脑卒中偏瘫患者二元应对水平有影响,经济收入越好,经济负担越小的夫妻,二元应对水平越高。患者及配偶职业对患者配偶二元应对水平有影响,患者为在职人员,二元应对水平相对较高。Objective To investigate the effect of economic status on the dyadic coping level of stroke patients with hemiplegia and their spouse.Methods From October 2020 to August 2021,619 inpatients with stroke hemiplegia and their spouses were investigated by general information questionnaire,economic status questionnaire and dyadic coping scale.Results A total of 243 cases(39.3%)of 619 patients and their spouses had a low level of dyadic coping,226 cases(36.5%)had a moderate level of dyadic coping,and 150 cases(24.2%)had dyadic coping levels;Univariate analysis showed that the patient's age,patient's education level,spouse's age,spouse's education level,marriage age,patient's occupational status,spouse's occupational status,average monthly household income,supporting their parents,raising their children,marital financial management were related to the dyadic coping level of stroke hemiplegic patients and their spouses,the difference was statistically significant(P<0.05).Multivariate analysis showed that patients with high monthly average income and non co management of finance had a relatively high level of binary coping(P<0.05);The level of dual coping of the spouses of patients whose occupational status was on-the-job was relatively high(P<0.05).Conclusion Stroke patients with hemiplegia and their spouse have a moderate level of dyadic coping;Economic status has an impact on the dyadic coping level of stroke hemiplegia patients and their spouse.The higher the economic income,the lower the economic burden of the couple,the higher the dyadic coping level.The occupation of patients and their spouses has an impact on the level of dual coping of patients'spouses.The patients are in-service personnel,and the level of dual coping is relatively high.
