银丹心脑通软胶囊治疗急性脑梗死的疗效及对血清P选择素、YKL-40水平的影响

摘　　要：目的分析银丹心脑通软胶囊治疗急性脑梗死的疗效及对血清P选择素、人类软骨糖蛋白39(HCgp-39/YKL-40)水平的影响。方法选取2018年1月至2019年12月在盘锦辽油宝石花医院接受治疗的100例急性脑梗死患者作为研究对象,采用抽签法分为对照组和试验组,各50例。其中,对照组给予常规治疗,试验组在对照组的基础上口服银丹心脑通软胶囊,每次3粒,每日3次,两组均连续治疗14 d。比较两组的临床疗效,血清P选择素、YKL-40、神经元特异性烯醇化酶(NSE)、神经营养因子(NTF)、肿瘤坏死因子-α(TNF-α)、高敏C反应蛋白(hs-CRP)水平和简易智力状态检查量表(MMSE)评分、Barthel指数变化及不良反应发生情况。结果试验组的总有效率高于对照组[90.0%(45/50)比66.0%(33/50)](P<0.01)。治疗后,两组血清P选择素、YKL-40水平均低于治疗前,且试验组低于对照组[(6.27±2.13)μg/L比(8.53±2.34)μg/L,(30.14±5.17)ng/L比(42.75±4.99)ng/L](均P<0.01)。治疗后,两组血清NSE水平均低于治疗前,且试验组低于对照组[(16.12±2.67)μg/L比(22.29±3.17)μg/L];NTF水平高于治疗前,且试验组高于对照组[(4.89±1.21)μg/L比(3.67±1.05)μg/L](P<0.01)。治疗后,两组血清TNF-α、hs-CRP水平均低于治疗前,且试验组低于对照组[(10.64±3.21)ng/L比(12.68±3.27)ng/L,(6.41±1.62)mg/L比(8.56±2.13)mg/L](均P<0.01)。治疗后,两组MMSE水平均低于治疗前,且试验组低于对照组[(5.13±1.41)分比(9.56±2.34)分];Barthel指数高于治疗前,且试验组高于对照组[(75.6±8.5)分比(63.5±7.2)分](均P<0.01)。试验组的总不良反应发生率为6.0%(3/50),对照组为16.0%(8/50),两组总不良反应发生率比较差异无统计学意义(χ^(2)=2.554,P=0.110)。结论急性脑梗死患者采用银丹心脑通软胶囊治疗效果显著,可能与其有效降低血清P选择素、YKL-40水平有关。Objective To study the effect of Yindan Xinnaotong soft capsule on acute cerebral infarction and its effect on levels of serum P-selectin and human cartilage glycoprotein-39(HCgp-39/YKL-40).Methods A total of 100 patients with acute cerebral infarction who received treatment in Panjin Liaoyou Baoshihua Hospital from Jan.2018 to Dec.2019 were included and divided into a control group and a test group by lottery method,with 50 patients in each group.The control group was given conventional treatment,and the test group was given Yindan Xinnaotong soft capsule,3 capsules per time,3 times a day,on the basis of the control group's regimen.Both groups were given continuous treatment for 14 days.The clinical efficacy,serum levels of P-selectin,YKL-40,neuron-specific enolase(NSE),neurotrophic factor(NTF),tumor necrosis factor-α(TNF-α),hypersensitive C-reactive protein(hs-CRP),mini-mental state examination(MMSE)score,Barthel index and incidence of adverse reactions of the two groups were compared.Results The total effective rate of the test group was higher than that of the control group[90.0%(45/50)vs 66.0%(33/50)](P<0.01).After treatment,the levels of serum P-selectin and YKL-40 in the two groups were lower than those before treatment,and the levels in the test group were lower than those in the control group[(6.27±2.13)μg/L vs(8.53±2.34)μg/L,(30.14±5.17)ng/L vs(42.75±4.99)ng/L](all P<0.01).After treatment,the serum NSE levels of the two groups were lower than those before treatment,and the test group was lower than that of the control group[(16.12±2.67)μg/L vs(22.29±3.17)μg/L];the serum NTF levels were higher than those before treatment,and the test group was higher than that in the control group[(4.89±1.21)μg/L vs(3.67±1.05)μg/L](P<0.01).After treatment,the levels of serum TNF-α and hs-CRP in the two groups were lower than those before treatment,and the levels in the test group were lower than those in the control group[(10.64±3.21)ng/L vs(12.68±3.27)ng/L,(6.41±1.62)mg/L vs(8.56±2.13)mg/L](all P<0.
