泪小管吻合术联合义管植入术治疗外伤性泪小管断裂的效果评价

摘　　要：目的:探讨用泪小管吻合术联合义管植入术治疗外伤性泪小管断裂的效果。方法:选取海南西部中心医院2020年1月至12月收治的60例外伤性泪小管断裂患者作为试验对象。随机将其分为讨论组与常模组,每组各有患者30例。常模组患者单独采用泪小管吻合术进行治疗,讨论组患者用泪小管吻合术联合义管植入术进行治疗。比较两组患者的临床疗效、术后并发症的发生率及眼表疾病指数量表(OSDI)的评分。结果:两组患者治疗的总有效率相比,P>0.05。讨论组患者的治愈率高于常模组患者,P<0.05。讨论组患者术后泪道再次阻塞的发生率低于常模组患者,P<0.05。术后6个月,讨论组患者OSDI中各项指标的评分均低于常模组患者,P<0.05。结论:用泪小管吻合术联合义管植入术治疗外伤性泪小管断裂的效果显著,术后患者的泪道能够达到解剖复位,泪道的通畅率高,且术后其泪小管再次阻塞的发生率低,有利于改善其眼部功能。Objective:To investigate the effect of lacrimal duct anastomosis combined with artificial duct implantation in the treatment of traumatic lacrimal duct rupture.Methods:A total of 60 patients with traumatic lacrimal canaliculi rupture admitted to Hainan West Central Hospital from January to December 2020 were selected as experimental subjects.They were randomly divided into discussion group and norm group,with 30 patients in each group.Patients in the normal group were treated with lacrimal duct anastomosis alone,and patients in the discussion group were treated with lacrimal duct anastomosis combined with artificial duct implantation.The clinical efficacy,incidence of postoperative complications and OSDI score were compared between the two groups.Results:The total effective rate of the two groups was compared,P>0.05.The cure rate of discussion group was higher than that of normal group,P<0.05.The incidence of reobstruction of lacrimal duct in discussion group was lower than that in normal group,P<0.05.Six months after surgery,the scores of OSDI indicators in the discussion group were lower than those in the normal group,P<0.05.Conclusion:Lacrimal duct anastomosis combined with artificial duct implantation is effective in the treatment of traumatic lacrimal duct rupture.Postoperative patients can achieve anatomical reduction of the lacrimal duct,high patency rate of the lacrimal duct,and low incidence of re-obstruction of the lacrimal duct after surgery,which is beneficial to improve the ocular function.
