摘　　要：目的观察改良胰腺空肠端侧套入式吻合术在预防胰十二指肠切除术后并发症胰瘘的临床效果。方法回顾性分析2015年1月至2021年12月行改良胰腺空肠端侧套入式吻合术76例患者的临床资料,观察患者术后胰瘘的发生情况。正态分布的计量资料以(-x±s)表示,偏态分布的计量资料以M(P25,P75)表示,计数资料以率表示。结果术后胰瘘39.5%(30/76),其中A级胰瘘为30.3%(23/76),B级胰瘘为9.2%(7/76),无C级胰瘘。结论改良的胰腺空肠端侧套入式吻合可以有效的减少严重胰瘘的发生,为胰十二指肠切除术后胰瘘的临床防治提供了有效的手段。Objective To observe the clinical effect of modified end-to-side pancreaticojejunostomy in preventing pancreatic fistula after pancreaticoduodenectomy.Methods The clinical data of 76 patients with modified pancreatic jejunum end-to-side invaginated pancreatic jejunostomy from January 2015 to December 2021 were retrospectively analyzed,and the incidence of postoperative pancreatic fistula was observed.Measurement data with normal distribution were represented as±s.Measurement data with skewed distribution were described as M(P25,P75).Enumeration data were described as rates.Results Postoperatively,there were 23 cases of grade A pancreatic fistula,7 cases of grade B pancreatic fistula,and 0 cases of grade C pancreatic fistula.T Postoperative pancreatic fistula was 39.5%(30/76),of which grade A was 30.3%(23/76),grade B was 9.2%(7/76),and grade C was 0%.Conclusion The modified end-to-side pancreaticojejunostomy can effectively reduce the occurrence of severe pancreatic fistula and provide an effective means for the clinical prevention and treatment of pancreatic fistula after pancreaticoduodenectomy.
