血清颗粒体蛋白检测在胃癌早期筛查中的应用研究

摘　　要：目的研究颗粒体蛋白(GRN)检测在胃癌诊断中的预测价值。方法纳入2019年1月-2020年12月收治的52例胃癌患者、52例胃良性病变患者和60例正常体检患者作为研究对象,空腹血液样本检测GRN、CEA、CA19-9、CA72-4、CA242、CA50水平,采用单因素、Logistic回归、ROC曲线分析比较血清GRN与常规肿瘤指标对胃癌患者的预测价值。结果单因素分析胃癌组的CEA、CA19-9、CA72-4、CA242、CA50、GRN均高于良性病变组和正常组,且差异均有统计学意义(P<0.05);Logistic回归分析整体模型具有统计学意义,(χ^(2)=67.376,P<0.001),6个项目差异均有统计学意义(P<0.05);ROC曲线分析CEA、CA19-9、CA72-4、CA50、GRN差异有统计学意义(P<0.05),线下面积分别为0.772、0.666、0.648、0.655、0.856,其中GRN的ROC线下面积最大,敏感度最高,为93.33%,特异度为73.3%。结论GRN在胃癌患者中呈现高表达,cut-off值为68.85 ng/L,对胃癌患者的预测价值优于本文中研究的其他5项常规肿瘤指标。Objective To study the predictive value of granular protein(GRN)detection in the diagnosis of gastric cancer.Methods A total of 52 patients with gastric cancer admitted from January 2019 to December 2020,52 patients with gastric benign lesions and 60 individuals with normal physical examination were included as the research objects.Fasting blood samples were conducted for the detection of GRN,CEA,CA19-9,CA72-4,CA242 and CA50 levels,and univariate,Logistic regression,ROC curve analysis were used to compare the predictive value of serum GRN and conventional tumor markers in gastric cancer patients.Results Univariate analysis showed that the CEA,CA19-9,CA72-4,CA242,CA50,GRN values of the gastric cancer group were higher than those in the benign lesion group and the normal group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that the whole model is statistically significant(χ^(2)=67.376,P<0.001),and the differences of 6 items are statistically significant(P<0.05).ROC curve analysis showed that the difference of CEA,CA19-9,CA72-4,CA50,GRN values is statistically significant(P<0.05),the area under curve(AUC)is 0.772,0.666,0.648,0.655,0.856 respectively,among which GRN has the largest AUC of ROC,the highest sensitivity(93.33%)and the highest specificity(73.3%).Conclusion GRN is highly expressed in gastric cancer patients and its cut-off value is 68.85 ng/L,which has better predictive value for gastric cancer patients than the other five conventional tumor indicators studied in this paper.
