摘 要:目的探究眼内血管生成素样蛋白4(ANGPTL4)的过度表达与视网膜中央静脉阻塞继发黄斑水肿的相关性。方法选取2016年9月〜2018年4月在青岛市海慈医院就诊的拟行玻璃体腔注射雷珠单抗治疗的38例继发于视网膜中央静脉阻塞的黄斑水肿老年患者为研究组,同期拟行白内障手术的45例老年白内障患者为对照组。测定两组病例房水中ANGPTL4和VEGF的浓度;荧光素眼底血管造影测量研究组视网膜毛细血管无灌注区面积,光学相干断层成像检查测量黄斑中央视网膜厚度和黄斑总体积。结果研究组房水ANGPTL4和血管内皮生长因子(VEGF)浓度高于对照组,差异均有统计学意义(P<0.05)。视网膜中央静脉阻塞患者房水中ANGPTL4浓度与视网膜毛细血管无灌注区面积、黄斑中央视网膜厚度及黄斑总体积均呈正相关(r=0.886、0.675、0.557,P<0.05)。在矫正了房水VEGF浓度后,研究组房水中ANGPTL4浓度与视网膜毛细血管无灌注区面积、黄斑中央视网膜厚度及黄斑总体积仍呈正相关。结论眼内ANGPTL4与视网膜中央静脉阻塞患者视网膜毛细血管无灌注区面积及黄斑水肿程度显著相关。Objective To investigate the relationship between the overexpression of intraocular angiopoietin-like protein 4(ANGPTL4) and macular edema secondary to central retinal vein occlusion.Methods From September 2016 to April 2018,38 older patients with macular edema secondary to central retinal vein occlusion who were treated by intravitreal injection of ranibizumab in Qingdao Hiser Hospital were selected as the research group.Forty-five senile cataract patients who underwent cataract surgery were selected as the control group.The concentrations of ANGPTL4 and vascular endothelial growth factor(VEGF) in the aqueous humor of the two groups were determined.Fluorescein fundus angiography was used to measure the area of non-perfused retinal capillaries in the research group.Optical coherence tomography was used to measure the central retinal thickness of macula and total macular volume.Results The concentrations of ANGPTL4 and VEGF in the aqueous humor of the research group were higher than those of the control group, and the differences were statistically significant(P<0.05).The concentration of ANGPTL4 in the aqueous humor of patients with central retinal vein occlusion was positively correlated with the area of non-perfused retinal capillaries, central retinal thickness of macula and total macular volume(r=0.886,0.675,0.557,P<0.05).After correction of VEGF concentration in the aqueous humor, the concentration of ANGPTL4 in aqueous humor was still positively correlated with the area of non-perfused retinal capillaries, central retinal thickness of macula and total macular volume.Conclusion Intraocular ANGPTL4 is significantly correlated with retinal capillary non-perfusion area and the degree of macular edema in patients with central retinal vein occlusion.
