上前牙即刻种植后即刻修复与使用愈合基台修复对患者治疗效果及牙龈红色美学效果的影响

摘　　要：目的对比上前牙即刻种植后即刻修复与使用愈合基台修复对患者治疗效果及牙龈红色美学效果的影响。方法采用回顾性分析方法,以2018年6月至2020年6月北京市中医药大学房山医院口腔科收治的68例上前牙即刻种植患者为研究对象,根据牙龈封闭方式分为研究组(n=34)与对照组(n=34)。研究组种植体植入后行临时义齿即刻修复,对照组种植体植入后行愈合基台封闭牙龈。比较两组不同时间段(修复完成时、修复后6个月、12个月)牙龈红色美学评分、种植体颈缘骨吸收量。结果研究组较对照组修复完成时牙龈红色美学评分更高,差异有统计学意义(P<0.05);两组修复后6个月、12个月时牙龈红色美学评分差异无统计学意义(P>0.05);较修复完成时,研究组修复后6个月、12个月时牙龈红色美学评分更低,差异均有统计学意义(P<0.05);对照组修复完成时与修复后6个月、12个月牙龈红色美学评分差异无统计学意义(P>0.05)。研究组修复完成时种植体颈缘骨吸收量明显低于对照组,差异有统计学意义(P<0.05);两组修复后6个月、12个月种植体颈缘骨吸收量差异无统计学意义(P>0.05);较修复完成时,两组修复后6个月、12个月种植体颈缘骨吸收量更高,差异均有统计学意义(P<0.05)。结论上前牙即刻种植后即刻修复与使用愈合基台均具有良好效果,均可恢复前牙区美观性与维持牙槽骨形态、高度,但即刻修复短期治疗效果更为显著,而随着时间延长愈合基台的牙龈红色美学效果更趋于稳定,临床医师应根据实际情况选取最佳修复方式。Objective To compare the aesthetic effect of immediate restoration after anterior teeth implantation with the use of healing abutment and the amount of bone absorption at the cervical margin of implants.Methods A total of 68 patients with immediate anterior tooth implantation were selected as the study subjects by retrospective analysis,who were admitted to Fangshan Hospital of Beijing University of Chinese Medicine from June 2018 to June 2020.They were divided into research group(n=34)and control group(n=34)according to gingival sealing methods.In the study group,temporary denture repair was performed immediately after implant implantation,while in the control group,healing abutment was performed after implant implantation.The two groups were compared at different time points(at the completion of the restoration,6 months and 12 months after the restoration).Results Compared with the control group,the aesthetic score of gingival red was higher in the study group,the difference was statistically significant(P<0.05).There was no significant difference in aesthetic score of gingival red between the two groups at 6 and 12 months after restoration(P>0.05).The aesthetic score of gingival red at 6 months and 12 months after the restoration was lower than that at the completion of the restoration,the difference was statistically significant(P<0.05).In the control group,there was no significant difference in the aesthetic score of gingival red at 6 and 12 months after the restoration(P>0.05).The amount of implant bone resorption in the study group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in implant bone uptake at 6 months and 12 months after restoration between the two groups(P>0.05).Compared with when the implant was completed,the implant cervical margin bone absorption was higher in both groups at 6 and 12 months after the implant was repaired,the difference was statistically significant(P<0.05).Conclusion Forward imme
