摘　　要：目的探讨ABCD^(3)评分联合数字减影血管造影(DSA)结果预测短暂性脑缺血发作(TIA)患者近期发生脑卒中的价值。方法纳入220例TIA患者,收集其临床资料,并对其进行ABCD^(3)评分和DSA检查。观察TIA患者入院后7 d内脑卒中的发生情况,采用Logistic回归模型分析ABCD^(3)评分分级、DSA结果与TIA患者近期发生脑卒中的相关性,应用受试者工作特征(ROC)曲线评估ABCD^(3)评分分级、DSA结果单独及二者联合预测TIA患者近期发生脑卒中的效果。结果220例患者中,有28例患者于入院后7 d内发生脑卒中,发生率为12.7%。多因素Logistic回归分析结果显示,ABCD^(3)评分分级、DSA结果均与TIA患者近期发生脑卒中具有相关性(均P<0.05)。ROC曲线分析结果显示,ABCD^(3)评分分级、DSA结果及二者联合预测TIA患者近期发生脑卒中的曲线下面积分别为0.779、0.782、0.854。结论ABCD^(3)评分联合DSA结果对TIA患者近期发生脑卒中具有一定的预测价值。Objective To explore the value of ABCD^(3) scoring combined with the findings of digital subtraction angiography(DSA)in predicting recent stroke seizure in patients with transient ischemic attack(TIA).Methods A total of 220 TIA patients were enrolled,and their clinical data were collected.In addition,ABCD^(3) scoring and DSA examinations were performed on the patients.The occurrence of stroke within seven days after admission was observed in TIA patients.The Logistic regression model was used to analyze the correlations of ABCD^(3) scoring grade and DSA findings with the occurrence of recent stroke in TIA patients.The receiver operating characteristic(ROC)curve was used to evaluate the efficacy of ABCD^(3) scoring grade,DSA findings,and their combination for predicting the occurrence of recent stroke in TIA patients.Results Among the 220 cases,28 cases developed stroke within seven days after admission,with an incidence of 12.7%.The results of multivariate Logistic regression analysis showed that both ABCD^(3) scoring grade and DSA findings correlated with the occurrence of recent stroke in TIA patients(all P<0.05).The results of ROC curve analysis showed that the areas under the curves of ABCD^(3) scoring grade,DSA findings,and their combination for predicting the occurrence of recent stroke in TIA patients were 0.779,0.782 and 0.854,respectively.Conclusion ABCD^(3) scoring combined with DSA findings has certain predictive value for recent stroke seizure in TIA patients.
