使用一次性包皮环切器、吻合器与传统手术治疗包皮过长及包茎的疗效比较

摘　　要：目的比较使用一次性环切器、吻合器及采用传统手术行包皮环切术的疗效。方法回顾性分析2019年6月至2021年6月在本院行包皮环切术的患者960例,根据术式的不同分为环切器组(578例)、吻合器组(234例)及传统方法组(148例),从手术时间、切口水肿率、外观满意度、并发症发生率等方面比较3种手术方式的疗效差异。结果环切器组、吻合器组、传统方法组的手术时间分别为(12.87±4.18)、(7.25±1.78)、(47.56±3.74)min,外观满意度分别为96.53%(558/578)、96.15%(225/234)、77.70%(115/148),术后包皮水肿率分别为70.93%(410/578)、41.03%(96/234)、1.35%(2/148),并发症发生率分别为6.06%(35/578)、22.65%(53/234)、0.68%(10/148)。环切器组、吻合器组的手术时间和外观满意度优于传统方法组,吻合器组、传统方法组的术后水肿率低于环切器组,传统方法组的术后并发症发生率低于环切器组、吻合器组。结论一次性环切器和吻合器具有操作简单、手术时间短、手术效果满意度高等特点,但对于病理性包茎及特殊病例采用传统环切术更具优势。Objective To compare the efficacy of using shang ring,stapler and traditional operation.Methods 960 patients who underwent circumcision in our hospital from June 2019 to June 2021 were retrospectively analyzed.They were divided into shang ring group(578 cases),stapler group(234 cases)and traditional method group(148 cases)according to different operation methods.The curative effects of the three operation methods were compared in terms of operation time,incision edema rate,appearance satisfaction and incidence of complications.Results The operation time of shang ring group,stapler group and traditional method group were(12.87±4.18),(7.25±1.78),(47.56±3.74)min respectively.The appearance satisfaction was 96.53%,96.15%and 77.70%respectively.The rates of postoperative prepuce edema were 70.93%,41.03%and 1.35%respectively.The incidence of complications were 6.06%,22.65%and 0.68%respectively.The operation time and appearance satisfaction rate of shang ring group and stapler group were better than those of traditional method group.The edema rate of stapler group and traditional method group was lower than that of shang ring group.The incidence of postoperative complications in the traditional method group was lower than that in the shang ring group and stapler group.Conclusion Using shang ring and stapler in the treatment of phimosis and redundant prepuce has the characteristics of simple operation,short operation time and satisfactory operation effect,but traditional circumcision has more advantages for pathological phimosis and special cases.
