摘　　要：目的探索经自然腔道内镜手术(NOTES)对直肠癌患者术后胃肠功能恢复及生活质量的影响。方法选取2018年11月至2021年1月在我院确诊为直肠癌患者共50例,用随机数字表法将患者分为观察组(25例,经自然腔道内镜手术)和对照组(25例,传统开腹手术),比较两组手术相关指标、术后胃肠功能恢复情况、生存状态和满意度。结果观察组手术时间、术中出血量和住院时间、肠蠕动恢复时间、排气时间和排便时间均低于对照组,差异有统计学意义(P<0.05);观察组术后的躯体疼痛、精神状态、健康状况的SF-36评分均高于对照组,差异有统计学意义(P<0.05);观察组术后出血量、并发症发生率和满意度较对照组无显著性差异,差异无统计学意义(P>0.05)。结论NOTES与传统开腹手术相比,对直肠癌患者术后胃肠功能恢复更有利,对术后生活质量影响更小,安全性更高,临床应用价值更高。Objective To explore the effect of natural orifice endoscopic surgery(NOTES)on postoperative gastrointestinal function recovery and quality of life in patients with rectal cancer.Methods A total of 50 patients with rectal cancer diagnosed in our hospital from November 2018 to January 2021 were selected,and the patients were divided into an observation group(25 cases,who underwent natural orifice endoscopic surgery)and a control group by random number table method.(25 cases,traditional laparotomy),compared the two groups of surgery-related indicators,postoperative gastrointestinal function recovery,survival status and satisfaction.Results The operation time,intraoperative blood loss and hospitalization time,bowel movement recovery time,exhaust time and defecation time in the observation group were all lower than those in the control group,and the differences were statistically significant(P<0.05).SF-36 score and satisfaction of mental state and health status were higher than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in postoperative bleeding volume,complication rate and satisfaction in the observation group compared with the control group(P>0.05).Conclusion Compared with traditional laparotomy,NOTES is more beneficial to the recovery of gastrointestinal function in patients with rectal cancer after surgery,has less impact on postoperative quality of life,higher safety,and higher clinical application value.
