摘 要:目的:比较高分辨率CT扫描与多层螺旋CT增强扫描检查在周围型肺癌诊断中的效能。方法:选取78例疑似周围型肺癌患者为研究对象,均进行多层螺旋CT增强扫描与高分辨率CT扫描检查,以病理检查结果为金标准,比较多层螺旋CT增强扫描与高分辨率CT扫描检查在周围型肺癌诊断中的效能及影像学征象检出率。结果:78例疑似患者中,周围型肺癌65例,占83.33%;高分辨率CT扫描检查诊断周围型肺癌的灵敏度、准确度均高于多层螺旋CT增强扫描检查,差异有统计学意义(P<0.05);多层螺旋CT增强扫描与高分辨率CT扫描检查诊断周围型肺癌的特异度、阳性预测值、阴性预测值比较,差异均无统计学意义(P>0.05);高分辨率CT扫描检查对分叶征、空气支气管征、空泡征、胸膜凹陷征、短毛刺征、棘突征、血管集束征的检出率均高于多层螺旋CT增强扫描检查,差异有统计学意义(P<0.05)。结论:高分辨率CT扫描检查诊断周围型肺癌的效能及影像学征象检出率高于多层螺旋CT增强扫描检查。Objective: To compare efficacies of high-resolution CT scan and multi-slice spiral CT scan in diagnosis of peripheral lung cancer. Methods: 78 patients with suspected peripheral lung cancer were selected as the research subjects, and all of them underwent enhanced multislice CT scan and high-resolution CT scan. Using the pathological examination results as the gold standard, the efficacies and the imaging signs detection rate of enhanced multi-slice spiral CT scan and high-resolution CT scan in the diagnosis of peripheral lung cancer were compared. Results: Among the 78 suspected patients, 65 were diagnosed as peripheral lung cancer, accounting for 83.33%. The sensitivity and accuracy of high-resolution CT scan in the diagnosis of peripheral lung cancer were higher than those of enhanced multi-slice spiral CT scan, and the differences were statistically significant(P<0.05).However, there were no significant differences in the specificity, positive predictive value and negative predictive value of enhanced multi-slice spiral CT scan and high-resolution CT scan in the diagnosis of peripheral lung cancer(P>0.05). The detection rates of lobulation sign, air bronchogram sign, vacuole sign,pleural depression sign, short spicule sign, spinous process sign and vascular bundle sign in high-resolution CT scan were higher than those in multi-slice spiral CT scan, and the differences were statistically significant(P<0.05). Conclusions: High-resolution CT scans have higher diagnostic performance and imaging features detection rate than multi-slice spiral CT scans in the diagnosis of peripheral lung cancer.
