柴胡疏肝散对非酒精性脂肪肝的治疗效果及机制探讨

摘　　要：目的探讨柴胡疏肝散对非酒精性脂肪肝的治疗效果及机制。方法选择将2020年6月~2021年6月我院收治的120例非酒精性脂肪肝患者,随机分为对照组、观察组,每组60例。对照组口服多烯磷脂酰胆碱胶囊,观察组给予经典方剂柴胡疏肝散。治疗时间均为60d。比较两组治疗前后血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、γ-谷氨酰转移酶(γ-GT)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、新饱食分子蛋白1(Nesfatin-1)、成纤维细胞生长因子21(FGF 21)水平。结果治疗后两组患者血清中TC、TG、ALT、AST、γ-GT、LDL-C、FGF 21水平均显著低于治疗前,且观察组上述各指标水平低于对照组;HDL-C、Nesfatin-1水平显著高于治疗前,且观察组高于对照组;差异均有统计学意义(P<0.05)。观察组总有效率80.70%,明显高于对照组(74.14%),两组比较差异有统计学意义(P<0.05)。结论柴胡疏肝散可有效调节非酒精性脂肪肝患者脂质代谢和肝功能,减轻FGF 21抵抗,改善机体对胰岛素的敏感性。Objective To investigate the therapeutic effect and mechanism of Chaihu Shugansan on nonalcoholic fatty liver disease.Methods A total of 120 patients with nonalcoholic fatty liver disease who were admitted to our hospital between June 2020 and June 2021 were included in this study.They were randomly divided into control group and observation group with 60 cases in each group.The control group was given polyene phosphatidylcholine capsules.The observation group was given classic Chaihu Shugansan.The treatment duration was 60 days.The clinical efficacy was evaluated,and the serum contents of ALT,AST,γ-GT,TC,TG,HDL-C and LDL-C were compared between the two groups before and after treatment.Serum levels of Nesfatin-1 and FGF 21 were compared between the two groups before and after treatment.Results After intervention,the contents of ALT,AST,γ-GT,HDL-C and FGF 21 in blood of the two groups were significantly lower than those before intervention,those contents in the observation group were significantly lower than that in the control group;The levels of HDL-C and Nesfatin-1 were significantly higher than those before intervention,those levels in the observation group were significantly higher than that in the control group(P<0.05).The total response rate in the observation group was significantly higher than that in the control group(80.70%vs 74.14%)(P<0.05).Conclusion Chaihu Shugansan can effectively regulate lipid metabolism and liver function in patients with nonalcoholic fatty liver disease,reduce resistance to FGF 21 and improve the body's sensitivity to insulin.
