摘　　要：目的系统评价呼吸肌训练对于脑卒中患者呼吸和平衡功能的临床疗效。方法检索Pubmed、Cochrane Library、Web of science、Embase、CBM、中国知网、万方医学、维普、临床试验注册中心中有关呼吸肌训练对脑卒中患者呼吸肌功能、平衡功能康复效果的随机对照试验。采用RevMan 5.3软件进行统计分析。结果共纳入12篇随机对照研究,包含样本量428例。Meta分析结果显示呼吸肌训练能明显增强患者最大吸气压MD=22.90,95%CI(19.58,26.21),P<0.001、最大呼气压MD=20.18,95%CI(12.03,28.33),P<0.001,改善脑卒中患者Berg平衡量表得分MD=5.07,95%CI(2.36,7.77),P<0.001。结论呼吸肌训练能够显著提升脑卒中患者的呼吸肌功能,一定程度上改善其平衡功能,但仍需高质量、大样本研究验证呼吸肌训练对脑卒中患者呼吸和平衡功能的有效性。Objective To explore the effect of respiratory muscle training on respiratory and balance function in patients with stroke.Method All randomized controlled trials(RCTs)on the respiratory and balance function,function rehabilitation effect of respiratory muscle training in patients with stroke were searched in the Pubmed,Cochrane Library,Web of science,EMbase,CBM,CNKI,WanFang,VIP database and Clinical Trails.RevMan 5.3 software was used for statistical analysis.Results A total of 12 studies were included with 428 patients.Meta analysis results showed that respiratory muscle training could significantly increase maximum inspiratory pressure[MD=22.90,95%CI(19.58,26.21),P<0.001],Maximum expiratory pressure[MD=20.18,95%CI(12.03,28.33),P<0.001],improve Berg balance[MD=5.07,95%CI(2.36,7.77),P<0.001].Conclusion Respiratory muscle training can significantly improve the respiratory and balance function in stroke patients.High-quality research with large sample is needed to examine its effectiveness.
