养血通络汤联合醒脑开窍针刺治疗肾虚血瘀型脑动脉硬化症患者的临床效果

摘　　要：目的探讨养血通络汤联合醒脑开窍针刺治疗肾虚血瘀型脑动脉硬化症患者的疗效。方法选取2019年3月至2020年6月辽阳辽化医院收治的68例脑动脉硬化症患者为研究对象,依据随机数字表法分为对照组与观察组,各34例。对照组给予醒脑开窍针刺治疗,观察组在对照组基础上采用醒脑开窍针刺干预,治疗8周。比较两组患者临床疗效、中医证候评分、血液流变学、炎症介质及血管内皮功能水平的变化。结果观察组治疗有效率为94.12%,明显高于对照组的79.41%(P<0.05);治疗后,观察组患者中医证候评分明显低于对照组,差异有统计学意义(P<0.05);治疗后,观察组血浆黏度、全血黏度低切、全血黏度高切、血细胞比容明显低于对照组(P<0.05);治疗后,观察组血清一氧化氮(NO)上升水平,内皮素-1(ET-1)、促血管生成素-2(Ang-2)下降,明显优于对照组(P<0.05);治疗后,观察组白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)明显低于对照组(P<0.05)。结论养血通络汤联合醒脑开窍针刺治疗肾虚血瘀型脑动脉硬化症临床效果良好,可有效改善患者的血液流变学、炎症介质及血管内皮功能。Objective To explore the curative effects of Yangxue Tongluo Decoction combined with Xingnao Kaiqiao acupuncture in the treatment of patients with cerebral arteriosclerosis of kidney deficiency and blood stasis.Methods From March 2019 to June 2020,a total of 68 patients with cerebral arteriosclerosis admitted to Liaoyang City’s Liaohua Hospital were selected as the research objects,and were randomly divided into a control group and a treatment group with 34 cases in each group.The control group was given Xingnao Kaiqiao acupuncture treatment.On the basis of the control group,the observation group was treated with Xingnao Kaiqiao acupuncture for 8 weeks.The changes in clinical efficacy,TCM syndrome integral,hemorheology,cytokines and vascular endothelial function levels were compared between the 2 groups.Results The effective rate of treatment in the observation group was 94.12%,which was significantly higher than 79.41%in the control group(P<0.05);after treatment,the TCM syndrome scores in the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05);After treatment,the plasma viscosity,whole blood viscosity low shear,whole blood viscosity high shear and hematocrit in the observation group were significantly lower than those in the control group(P<0.05).The decreased levels of ET-1 and angiopoietin-2(Ang-2)in the observation group were significantly better than those in the control group(P<0.05).Sensitive C-reactive protein(hs-CRP)was significantly lower than that in the control group(P<0.05).Conclusion Yangxue Tongluo Decoction combined with Xingnao Kaiqiao acupuncture has a good clinical effect in treating cerebral arteriosclerosis of kidney deficiency and blood stasis type,and can effectively improve the patient's hemorheology,cytokines and vascular endothelial function.
