摘 要:目的探讨CT能谱成像技术在腮腺良恶性肿瘤鉴别诊断中的应用价值.方法选取腮腺肿瘤患者55例(良性肿瘤40例,恶性肿瘤15例),所有患者均行能谱CT平扫及双期增强扫描,获得病灶碘浓度(IC)及40 keV、70 keV、100 keV、140 keV单能量CT值,并计算分段能谱曲线斜率.结果腮腺恶性肿瘤的静脉期40 keV及70 keV单能量CT值、各段能谱曲线斜率及IC均高于良性肿瘤,差异有统计学意义(P<0.05).动脉期腺淋巴瘤单能量CT值最高,多形性腺瘤最低(P<0.05);静脉期40 keV、70 keV恶性肿瘤单能量CT值最高(P<0.05),腺淋巴瘤与多形性腺瘤差异无统计学意义(P>0.05),100 keV、140 keV多形性腺瘤单能量CT值最低(P<0.05),恶性肿瘤与腺淋巴瘤差异无统计学意义(P>0.05).动脉期40~70 keV段能谱曲线斜率腺淋巴瘤最高,多形性腺瘤最低(P<0.05);静脉期恶性肿瘤40~70 keV、70~100 keV及100~140 keV段能谱曲线斜率均最高(P<0.05),腺淋巴瘤与多形性腺瘤差异无统计学意义(P>0.05).动脉期多形性腺瘤IC最低(P<0.05),腺淋巴瘤与恶性肿瘤差异无统计学意义(P>0.05);静脉期恶性肿瘤IC最高,腺淋巴瘤最低(P<0.05).结论不同病理类型腮腺肿瘤的能谱CT单能量CT值、分段能谱曲线斜率及IC存在差异,利用能谱CT成像技术有助于腮腺肿瘤的术前鉴别诊断.Objective To explore the value of CT spectrum imaging technology in differential diagnosis of parotid benign and malignant tumors.Methods A total of 55 patients with parotid tumors(40 cases of benign tumors and 15 cases of malignant tumors)were collected.All patients underwent energy spectrum CT plain scan and dual-phase enhanced scan.The iodine concentration(IC),the monochromatic CT value of 40 keV,70 keV,100 keV and 140 keV,and the slope of the segmented energy spectrum curve were all measured and analyzed.Results The monochromatic CT value of 40 keV and 70 keV,curve slope and IC in venous phase of the malignant tumors were all higher than those of the benign tumors,and there was statistically significant(P<0.05).The monochromatic CT value in arterial phase was the highest in warthin’s tumor and that of pleomorphic adenoma was the lowest(P<0.05).The monochromatic CT value of 40 keV and 70 keV in venous phase was the highest in malignant tumor(P<0.05),and there was no statistical significance difference between warthin’s tumor and pleomorphic adenoma(P>0.05).The monochromatic CT value of 100 keV and 140 keV in venous phase was the lowest in pleomorphic adenoma(P<0.05),and there was no statistically significant between malignant tumor and warthin’s tumor(P>0.05).The curve slope of 40-70 keV in arterial phase was the highest in warthin’s tumor and that of pleomorphic adenoma was the lowest(P<0.05).The curve slope of 40-70 keV,70-100 keV,100-140 keV in venous phase were the highest in malignant tumor(P<0.05),and there was no significant difference between warthin’s tumor and pleomorphic adenoma(P>0.05).The IC in arterial phase was the lowest in pleomorphic adenoma(P<0.05),there was no significant difference between warthin’s tumor and malignant tumor(P>0.05).The IC in venous phase was the lowest in warthin’s tumor and that of malignant tumor was the highest(P<0.05).Conclusion Different pathological types of parotid tumors have different monochromatic CT values,segmented energy spectrum curve slopes an
