疏肝益肾中药治疗乳腺癌类围绝经期综合征疗效Meta分析

摘　　要：目的系统评价疏肝益肾中药治疗乳腺癌类围绝经期综合征的疗效。方法计算机检索中国知网(CNKI)、万方、维普、PubMed、The Cochrane Library数据库,搜索疏肝益肾中药治疗乳腺癌类围绝经期综合征的随机对照试验(randomized controlled trial,RCT),检索时间为各数据库建库至2020年12月31日。由两名工作人员分别根据纳入标准进行文献检索、筛选和数据的提取,最后评估所筛选研究的偏倚风险,统计学软件选用RevMan5.4。结果共纳入13个RCT,976例患者。Meta分析结果显示:治疗组在改善类围绝经期症状积分[MD=-13.19,95%CI(-15.58,-10.79),P<0.00001]、总有效率[RR=1.99,95%CI(1.41,2.82),P<0.0001]、生活质量评分(KPS)[MD=5.24,95%CI(2.97,7.50),P<0.00001]、调节T细胞亚群[MD=0.69,95%CI(0.60,0.78),P<0.00001]等方面较对照组存在优势,差异有统计学意义(P<0.05);治疗组在调节雌激素水平、改善子宫内膜厚度方面较对照组无明显优势,差异无统计学意义(P>0.05)。结论根据Meta分析结果,疏肝益肾类中药可显著缓解乳腺癌患者相关类围绝经期综合征症状,提高治疗有效率,改善生活质量,提高机体免疫功能,但对人体雌激素水平和子宫内膜厚度的影响较小。由于所纳入的文献质量较低、包含的病例数较少,以上所得结论尚需要更多高质量RCT予以证明。Objective To systematically evaluate the efficacy of soothing liver and tonifying kidney herbs on breast cancer with perimenopausal syndrome.Methods Databases including CNKI,WanFang,VIP,PubMed and the Cochrane library were searched to collect RCTs of soothing liver and tonifying kidney herbs on breast cancer with perimenopausal syndrome from the database establishment time to December 31,2020.Two researchers screened the literatures,extracted the data,assessed risk of bias of the included literatures,and analyzed the data with Revman 5.4 statistical software.Results A total of 13 RCTs and976 patients were included.Meta-analysis showed that:compared with control group,soothing liver and tonifying kidney herbs group adjustment Kupperman scores[MD=-13.19,95%CI(-15.58,-10.79),P<0.00001]and T cell subsets[MD=0.69,95%CI(0.60,0.78),P<0.00001],improved treatment efficiency[RR=1.99,95%CI(1.41,2.82),P<0.0001]and KPS score[MD=5.24,95%CI(2.97,7.50),P<0.00001].However,there was no statistical difference between two groups in sex hormone levels and endometrial thickness.Conclusion According to the results of Meta-analysis,soothing liver and tonifying kidney herbs can adjustment Kupperman scores and T cell subsets,improved treatment efficiency and KPS score,and no effect on sex hormone levels and endometrial thickness.However,due to the low quality of the included studies and small sample size,the above conclusions are still required to be verified by more highquality randomized controlled trials.
